摘要
目的了解吉林省医疗机构软式内镜消毒再处理操作流程的现况。方法于2019年12月采用自行设计软式内镜消毒剂使用情况调查问卷,基于吉林省研究型医院学会内镜护理与感染控制专业委员会平台,调查吉林省78所医疗机构软式内镜消毒剂使用及浓度监测现状。结果共发放调查问卷78份,回收有效问卷78份,有效回收率100%。78家医疗机构拥有1~167条内镜,消毒槽1~30个,日诊疗量0~320例。69.20%的医疗机构使用邻苯二甲醛。53.84%的医疗机构由专职内镜护理人员负责消毒剂管理工作。68.35%的医疗机构对于《WS-5072016软式内镜清洗消毒技术规范》中消毒剂浓度监测条目的理解为“每天工作开始前进行消毒剂浓度监测”;在实际工作中,67.95%的医疗机构每天上午工作前检测1次,58.97%的医疗机构根据消毒剂浓度检测结果更换新的消毒剂。吉林省开展软式内镜诊疗项目的医疗机构使用的消毒剂(灭菌剂)共涉及15个品牌,其中仅有5个品牌强调需消毒剂有效浓度监测;重复使用消毒剂或灭菌剂有效浓度检测执行合格率为36.90%。62.82%的医院采用手写记录重复使用消毒剂浓度监测情况。结论吉林省医疗机构对于消毒剂浓度监测频率的确定、检测流程的执行存在较大差异,对于记录内容及检测后试纸的保存方式也尚未达成一致。
OBJECTIVE To understand the current status of recycled procedures for disinfection of flexible endoscopes in medical institutions in Jilin Province.METHODS Questionnaire was designed for the status of use of flexible endoscopes in Dec,2019.The current status of use of disinfectants for flexible endoscopes and monitoring of concentrations in 78 medical institutions of Jilin Province were investigated based on platform of endoscopic care and infection control committee of Jilin Province Research-oriented Hospital Society.RESULTS A total of 78 questionnaires were sent out,and 78 valid ones were recycled,with the effective recycle rate 100%.The 78 medical institutions had 1-167 endoscopes,1-30 disinfection tanks,and 0-320 cases of daily diagnosis and treatment.69.20%of the medical institutions used phthalaldehyde.53.84%of the medical institutions had the full-time personnel to take charge for management of disinfectants.68.35%of the medical institutions interpreted the disinfectant concentration monitoring items in"WS-5072016 technical specifications for cleaning and disinfection of flexible endoscope"as"monitor the disinfectant concentration before work every day".In the actual work,67.95%of the medical institutions tested once every morning before work,and 58.97%of the medical institutions changed disinfectants according to result of disinfectant concentration test.Totally 15 brands of disinfectants(sterilizing agents)were used in the medical institutions of Jilin Province,5 of which stressed on the monitoring of the effective concentration of disinfectants;the qualified rate of monitoring of effective concentrations of recycled disinfectants or sterilizing agents was 36.90%.62.82%of the medical institutions manually recorded the monitoring status of concentrations of the recycled disinfectants.CONCLUSION There are significant differences in the monitoring frequency of concentration of disinfectant and execution of test procedures among the medical institutions in Jilin Province.The medical institutions have not re
