摘要
目的研究叉头翼状螺旋转录因子3(Foxp3)、辅助性T细胞17(Th17)、Toll样受体(TLR)在乙肝病毒携带孕妇外周血单个核细胞中的表达。方法选取2018年2月-2021年2月于南京医科大学附属妇产医院建册、产检且乙肝表面抗原(HBsAg)阳性孕妇60例为研究组,选取同时期进行建册、产检且HBsAg阴性孕妇60名为对照组,检测两组孕妇外周血T淋巴细胞、Foxp3、Th17和Toll样受体的表达情况。结果研究组CD_(4)^(+)水平较对照组降低,CD_(8)^(+)水平升高(P<0.05);研究组Foxp3、TLR4、TLR7表达水平高于对照组,Th17表达水平低于对照组(P<0.05);不同乙肝病毒载量Foxp3、Th17、TLR4、TLR7水平比较差异有统计学意义(P<0.05);高载量组孕妇Foxp3、TLR4、TLR7水平高于中、低载量组,Th17水平低于中、低载量组(P<0.05);Spearman秩相关分析结果显示,乙肝病毒载量与Foxp3、TLR4、TLR7水平均呈正相关,与Th17水平呈负相关(P<0.05)。结论乙肝病毒携带孕妇Foxp3、Th17、TLR4、TLR7表达异常,临床应重视高病毒载量孕妇上述指标的检测,有效控制病毒的复制及免疫的平衡。
OBJECTIVE To observe the expressions of forkhead or winged helix transcription 3(Foxp3),helper T cell 17(Th17)and Toll-like receptors(TLR)in peripheral blood mononuclear cells of pregnant women with hepatitis B virus(HBV).METHODS From Feb 2018 to Feb 2021,a total of 60 pregnant women who were profiled,received prenatal examination and tested positive for hepatitis B surface antigen(HBsAg)in Obstetrics and Gynecology Hospital of Nanjing Medical University were assigned as the study group,meanwhile,60 pregnant women who were profiled,received prenatal examination and tested negative for HBsAg were chosen as the control group.The expression levels of peripheral blood T lymphocytes,Foxp3,Th17 and Toll-like receptors were detected for the two groups of pregnant women.RESULTS The CD_(4)^(+)level of the study group was lower than that of the control group,while the CD_(8)^(+)level of the study group was higher than that of the control group(P<0.05).The expression levels of Foxp3,TLR4 and TLR7 of the study group were higher than those of the control group,while the Th17 level of the study group was lower than that of the control group(P<0.05).There were significant differences in the expression levels of Foxp3,Th17,TLR4 and TLR7 among the patients with different HBV loads(P<0.05).The expression levels of Foxp3,TLR4 and TLR7 of the high-load group were higher than those of the medium-load group and the low-load group,while the Th17 level of the high-load group was lower than that of the medium-load group and the low-load group(P<0.05).The Spearman rank correlation analysis showed that the HBV load was positively correlated with the levels of Foxp3,TLR4 and TLR7,but it was negatively correlated with the Th17 level(P<0.05).CONCLUSION The pregnant women carrying with HBV show abnormal expressions of Foxp3,Th17,TLR4 and TLR7.It is necessary for the hospital to pay great attention to the detection of the above indexes for the pregnant women with high-load HBV so as to effectively control the viral replication and immunologic b
