摘要
目的观察富血小板血浆(PRP)对糖尿病大鼠难愈感染创面的影响并分析作用机制。方法将24只SD雄性大鼠给予高脂高糖饲养+腹腔注射链脲佐菌素(STZ)制备糖尿病模型,其中成功20只,随机分为模型组和治疗组各10只,另取大鼠设对照组和取血组各10只,取腹主动脉血制备富血小板血浆,其余三组建立皮肤缺损创伤模型,干预14 d后,观察大鼠一般情况,比较随机血糖、创面愈合率,痂下组织菌落数,以及创面组织中肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、IL-6和羟脯氨酸(HYP)水平,血管内皮生长因子(VEGF)、转化生长因子-β1(TGF-β1)、Smad同源物3(Smad3)相对表达量。结果与同时间对照组比较,模型组、治疗组血糖均上升(P<0.05),治疗组治疗第14天血糖低于治疗前(P<0.05);治疗第7、14天,与对照组比较,模型组大鼠创面愈合率降低(P<0.05),与模型组比较,治疗组大鼠创面愈合率显著升高(P<0.05);模型组第3、7、14天创面菌落数均高于对照组(P<0.05),治疗组各时间创面菌落数均低于模型组(P<0.05);与模型组比较,治疗组创面组织中3种炎性因水平均降低(P<0.05),HYP水平上升(P<0.05),VEGF、TGF-β1、Smad3相对表达量上调(P<0.05)。结论PRP能减少糖尿病大鼠难愈感染创面菌落数,减轻炎症反应,促进创面愈合,可能与通过激活TGF-β1/Smad3信号通路,调控VEGF表达,诱导血管再生有关。
OBJECTIVE To observe the effect of platelet-rich plasma(PRP)on refractory infected wounds of rats with diabetes mellitus and analyze the mechanisms of action.METHODS Totally 24 SD male rats were given high-fat and high-glucose feeding plus intraperitoneal injection of streptozotocin(STZ)to prepare diabetes mellitus models.Among them,20 rats were successfully molded and randomly divided into the model group with 10 rats and the treatment group with 10 rats,10 rats were chosen as the control group,and 10 rats were chosen as the blood collection group.The abdominal aortic blood specimens were collected from the blood collection group to prepare PRP,and the skin defect injury models were established in other three groups.The general status of the rats was observed after the intervention for 14 days;the random blood glucose,wound healing rate and bacterial colony counts of subscab tissues were compared among the groups,the levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin 6(IL-6)and hydroxyproline(HYP)in wound tissues were compared among the groups,and the relative expression levels of vascular endothelial growth factor(VEGF),transforming growth factor-β1(TGF-β1)and Smad3 were compared among the groups.RESULTS The blood glucose level of the model group and the treatment group was higher than that of the control group at the same time points(P<0.05);the blood glucose level of the treatment group was lower after the treatment for 14 days than before the treatment(P<0.05).The wound healing rate of the model group was lower than that of the control group on Day 7 and Day 14 of treatment(P<0.05);the wound healing rate of the treatment group was remarkably higher than that of the model group(P<0.05).The bacterial colony counts of the model group were more than those of the control group on Day 3,7 and 14(P<0.05);the bacterial colony counts of wounds of the treatment group were less than those of the model group at the time points(P<0.05).The levels of three inflammatory factors in wound tissue
