摘要
目的:探究体验式学习干预联合低碳水化合物膳食干预对2型糖尿病(T2DM)超重患者疾病知识及血糖水平的影响。方法:选取2020年7月至2021年2月于某院就诊的112例T2DM超重患者为研究对象,采用随机数字法将其分为实验组和对照组,各56例。对照组给予低碳水化合物膳食干预,实验组在对照组的基础上联合体验式学习干预。统计比较两组干预前后血糖水平、糖尿病知识水平得分、自我管理水平得分及体重和腰围。结果:干预前,两组的血糖水平、糖尿病知识水平得分、自我管理水平得分及体重和腰围均无显著差异(P>0.05);干预后,实验组空腹血糖(FPG)、餐后2h血糖(2hPG)和糖化血红蛋白质(HbAlc)水平均明显低于对照组(t=6.223,5.801,5.533;P<0.05),糖尿病知识水平得分和糖尿病自我管理行为量表(SDSCA)饮食管理、运动管理、血糖监测、足部护理、用药管理得分均明显高于对照组(t=9.198,6.926,7.197,6.200,5.015,7.567;P<0.05),体重和腰围均明显小于对照组(t=3.410,2.947;P<0.05)。结论:体验式学习干预联合低碳水化合物膳食干预可有效控制T2DM超重患者血糖水平,提高患者的疾病知识水平和自我管理能力,此外还可协助患者减重,短期效果良好。
Objective:To observe the influence of experiential learning intervention combined with low-carbohydrate dietary intervention on disease knowledge level and blood glucose level of type 2 diabetes mellitus(T2DM)overweight patients.Methods:A total of 112 T2DM overweight patients who were admitted to our hospital from July 2020 to February 2021 were selected as study objects and divided into experimental group and control group by ran dom number method,with 56 cases in each group.The control group was treated with low-carbohydrate dietary intervention,while the experimental group was treated with experiential learning intervention on the basis of the treatment of the control group.Blood glucose level,diabetes mellitus knowledge level score,self-management level score,weight and waist circumference were statistically compared between the two groups before and after intervention.Results:After intervention,levels of fasting plasma glucose(FPG),2-hour postprandial blood glucose(2hPG)and glycosylated hemoglobin(HbAlc)in the experimental group were significantly lower than those in the control group(t=6.223,5.801,5.533,all P<0.05);scores of diabetes mellitus knowledge level and scores of diet management,exercise management,blood glucose monitoring,foot care and medication management in summary of diabetes self-care activities(SDSCA)of the experimental group were significantly higher than those of the control group(t=9.198,6.926,7.197,6.200,5.015,7.567,all P<0.05);and the weight and waist circumference of the experimental group were significantly lower than those of the control group(t=3.410,2.947,all P<0.05).Conclusion:The experiential learning intervention combined with low-carbohydrate dietary intervention can control the blood glucose level of the T2DM overweight patients and raise their disease knowledge level and self-management ability.In addition,it can also help patients lose weight with good short-term effect.
