摘要
目的探讨血清胱抑素C(CysC)、转化生长因子-β1(TGF-β1)对老年腔隙性脑梗死(LI)患者早期神经功能恶化(END)的预测价值。方法选取2020年1月至2021年12月琼海市人民医院神经内科收治的老年LI患者124例,采用多因素Logistic回归分析老年LI患者发生END的危险因素,分析血清CysC、TGF-β1对END的早期预测价值。结果124例老年LI患者中发生END患者28例,发生率为22.58%。END组患者入院时NIHSS评分、病灶累及内囊后肢患者比例及血清CysC水平明显高于非END组,而END组患者血清TGF-β1水平则低于非END组(P<0.05)。Logistic回归分析显示,NIHSS评分、病灶累及内囊后肢、血清CysC及TGF-β1水平是LI患者发生END的危险因素(P<0.05)。CysC联合TGF-β1检测对LI患者发生END预测的曲线下面积为0.867(95%CI为0.791~0.943),灵敏度为85.7%,特异度为78.1%。结论老年LI患者END发生率较高,可能与NIHSS评分高、病灶累及内囊后肢、血清CysC升高和TGF-β1降低有关,检测血清CysC、TGF-β1水平有助于早期预测END的发生。
Objective To investigate the predictive value of serum cystatin C(CysC)and transforming growth factor-β1(TGF-β1)on early neurological deterioration(END)in senile patients with lacunar infarction(LI).Methods 124 elderly LI patients admitted to the Neurology Department of Qionghai People’s Hospital from January2020 to December 2021 were selected.The risk factors of END in elderly LI patients were analyzed by multivariate Logistic regression.The early predictive value of serum CysC and TGF-β1 on END was analyzed.Results 28cases of 124 elderly LI patients developed END,with the incidence was 22.58%.NIHSS score,the proportion of lesions involving inner capsule hind limb and serum CysC levels in END group were significantly higher than those in non-END group,while the serum TGF-β1 levels in END group was significantly lower than that in non-END group(P<0.05).Logistic regression analysis showed that NIHSS score,lesion involving inner capsule hind limb,serum CysC and TGF-β1 levels were the risk factors for END in LI patients(P<0.05).The area under the curve of CysC combined with TGF-β1 for predicting END in LI patients was 0.867(95%CI:0.791-0.943),the sensitivity was 85.7%and the specificity was 78.1%.Conclusion The high incidence of END in elderly LI patients may be related to the high NIHSS score,lesion involving inner capsule hind limb,and the increased of CysC and decreased of TGF-β1.Detection of the serum CysC and TGF-β1 levels is helpful to early predict the occurrence of END.
