摘要
目的 观察重组人生长激素(recombinant human growth hormone,r-hGH)联合助长推拿治疗矮小症的效果及对脂肪因子的影响。方法 选择2018年1月至2019年12月上海中医药大学附属第七人民医院儿科门诊收治的特发性矮小症患儿96例,根据随机数字表法分为观察组和对照组,每组48例。对照组给予r-hGH,观察组在对照组治疗基础上联合助长推拿。对比治疗后两组临床疗效、身高标准差积分(height standard deviation score,HtSDS)、生长速度(growth velocity,GV)、预测成年身高(prediction of adult height,PAH)差值、空腹血糖(fasting blood glucose,FBG)、空腹胰岛素(fasting insulin,FINS)、稳态模型胰岛素抵抗指数(homeostasis model insulin resistance index,HOMA-IR)、血清骨钙素(osteocalcin,OCN)、25羟维生素D[25(OH)D]、胰岛素样生长因子(insulin-like growth factor,IGF)-1、瘦素、脂联素和内脂素水平的变化。结果 观察组临床总有效率高于对照组(85.4%vs 62.50%,P <0.05)。治疗后,两组血清瘦素和内脂素水平较治疗前显著降低(P <0.01),且观察组较对照组降低更显著(P <0.01)。两组HtSDS、GV、PAH差值、FBG、FINS、HOMA-IR、OCN、25(OH)D、IGF-1和脂联素水平较治疗前显著升高(P <0.01),且观察组HtSDS、GV、PAH差值、OCN、25(OH)D、IGF-1和脂联素升高水平较对照组更显著(P <0.01),对照组FBG、FINS、HOMA-IR升高水平较观察组更显著(P <0.01)。结论 r-hGH联合助长推拿治疗矮小症效果显著,可明显提高生长速度,降低IR,调节骨质代谢,可能与其可缓解脂肪因子紊乱有关。
Objective To observe the curative effects of recombinant human growth hormone(r-hGH) in the treatment of short stature and the effects of massage therapy on adipokines. Methods A total of 96 children with idiopathic short stature admitted to the department of pediatrics, seventh people’s hospital affiliated to Shanghai university of traditional Chinese medicine from January 2018 to December 2019 were selected and divided into observation group and control group according to random number method, with 48 cases in each group. The control group was given r-hGH, and the observation group was combined with promoting massage on the basis of the control group. The clinical efficacy, height standard deviation score(HtSDS), growth velocity(GV), difference value of prediction of adult height(PAH), fasting blood glucose(FBG), fasting insulin(FINS), homeostasis model insulin resistance index(HOMA-IR), serum osteocalcin(OCN),25 hydroxyvitamin D[25(OH)D], insulin-like growth factor(IGF)-1, leptin, adiponectin and endolipin levels. Results The total effective rate of the observation group was higher than that of the control group(85.4% vs 62.50%, P<0.05).After treatment, the levels of serum leptin and endolipin in both groups were significantly decreased compared with those before treatment(P<0.01), and the levels in the observation group were significantly decreased compared with the control group(P<0.01). HtSDS, GV, difference value of PAH, FBG, FINS, HOMA-IR, OCN, 25(OH)D, IGF-1 and adiponectin levels in two groups were significantly increased compared with those before treatment(P<0.01). The increased levels of HtSDS, GV, PAH difference, OCN, 25(OH)D, IGF-1 and adiponectin in the observation group were more significant than those in the control group(P<0.01), and the increased levels of FBG, FINS and HOMA-IR in the control group were more significant than those in the observation group(P<0.01). Conclusion The combination of r-hGH and massage can significantly improve the growth rate, reduce IR and regulate bone metabolism
