摘要
目的:探讨蜂针治疗联合药物外敷对类风湿性关节炎活动期患者的临床疗效、内循环状态及炎症因子的影响。方法:选择2019年1月—2020年10月在海南省海口市中医医院接诊的84例类风湿性关节炎活动期患者以随机数字表法分为两组各42例。对照组给予常规西医治疗,观察组在此基础上加以蜂针联合药物外敷治疗。比较两组患者临床疗效、临床症状、炎症指标、微循环状态及不良反应。结果:观察组总有效率95.24%(40/42)高于对照组80.95%(34/42),差异有统计学意义(P<0.05)。治疗后两组患者关节肿胀度、关节疼痛度、晨僵时间、CRP、血沉、类风湿因子及血管阻力均下降,且观察组均低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者收缩期峰值速度、舒张期峰值速度均升高,且观察组均高于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率7.14%(3/42),对照组不良反应发生率4.76%(2/42),恶心1例,两组差异无统计学意义(P>0.05)。结论:常规西医治疗基础上加以蜂针联合药物外敷治疗较单独采用常规西医治疗可提高类风湿性关节炎活动期患者疗效,缓解患者症状,降低炎症指标并改善微循环状态。
Objective:To investigate the clinical efficacy of bee venom acupuncture combined with medicinal external application in treatment of active rheumatoid arthritis(RA),and to explore its influence to internal circulation status and inflammatory factors.Methods:84 patients with active RN were randomly divided into the control group and the observation group,with 42 cases in each group.The control group was treated with conventional western medicine treatment;on which basis,the observation group was also treated with bee venom acupuncture combined with medicinal external application.Clinical efficacy,clinical symptoms,inflammatory indicators,microcirculation status and adverse reactions were compared between the two groups.Results:The total effective rate of the observation group was 95.24%(40/42),which was higher than 80.95%(34/42)of the control group(P<0.05).The joint swelling degree,the joint pain degree,the morning stiffness time,the CRP,the erythrocyte sedimentation rate,the rheumatoid factor and the vascular resistance were all decreased after the treatment compared with those before the treatment in the two groups(P<0.05),and the decreases in the observation group were more significant than those in the control group(P<0.05).The velocities of peak systolic and diastolic peak were increased after the treatment compared with those before the treatment in the two groups(P<0.05),and the increases in the observation group were more significant than those in the control group(P<0.05).The adverse reaction rate was 7.14%(3/42)in the observation group,and it was 4.76%(2/42)in the control group;in which there was no statistical difference between the two groups(P>0.05).Conclusion:Bee venom acupuncture combined with medicinal external application and conventional western medicine can improve the curative effect,relieve symptoms,reduce inflammation and improve microcirculation in treatment of active RN.The combined therapy is better than the single conventional western medicine therapy.
