泄浊通腑方保留灌肠对维持性血液透析患者IS、PCS、TMAO的影响 被引量:1

摘要 目的观察泄浊通腑方保留灌肠对维持性血液透析患者肠源性尿毒素硫酸吲哚酚(IS)、硫酸对甲酚(PCS)、氧化三甲胺(TMAO)及临床症状的影响。方法维持性血液透析患者60例,随机分为观察组和对照组。2组均用优质蛋白饮食及药物,规律血液透析等综合治疗,在此基础上,观察组用泄浊通腑方100 mL保留灌肠,对照组用肥皂液100 mL保留灌肠。共观察8周,入组时、治疗第4周及第8周完成抽血及随访,分别观察2组治疗前后临床症状及IS、PCS、TMAO指标等。结果泄浊通腑方保留灌肠对血液透析患者临床症状缓解及IS、PCS、TMAO指标改善明显优于肥皂液保留灌肠,差异有统计学意义(P<0.05)。结论泄浊通腑方保留灌肠对于血液透析患者IS、PCS、TMAO具有良好的清除作用,可能进一步减少维持性血液透析患者心血管事件的发生。以上这些作用可能与调节血液透析患者的肠道菌群相关。 Objective To observe the effect of Xiezhuotongfu Formula retention enema on maintenance hemodialysis patients with intestinal-derived uremic toxins indoxyl sulfate(IS),p-cresyl sulfate(PCS),trimethylamine oxide(trimethylamine N-oxide,TMAO)and clinical symptoms.Methods 60 maintenance hemodialysis patients were randomly divided into the treatment group and the control group.Both groups were treated with high-quality protein diet and drugs,regular hemodialysis and other comprehensive treatments.On this basis,the treatment group was given retention enema with 100mL retention enema with Xiezhuotongfu Formula,and the control group was given 100 mL soap solution.Results A total of 8 weeks of observation were performed.Blood draws and follow-up were completed at the time of the enrollment,the 4th and 8th weeks of the treatment,and the clinical symptoms,IS,PCS,and TMAO indexes of the two groups before and after treatment were observed respectively.Results Xiezhuotongfu Formula retention enema was significantly better than soap solution retention enema in relieving clinical symptoms and improving IS,PCS and TMAO indexes of hemodialysis patients,with statistical significance(P<0.05).Conclusion Xiezhuotongfu Formula retention enema has a good clearing effect on IS,PCS and TMAO in hemodialysis patients,and may further reduce the occurrence of cardiovascular events in maintenance hemodialysis patients.These effects may be related to the regulation of gut microbiota in hemodialysis patients.
