全自动血细胞分析仪报警值系统对血常规复片工作的影响 被引量:1

摘要 目的 探讨全自动血细胞分析仪报警值与显微镜镜检结果的关系及应用价值。方法 收集2022年2月15日至3月16日医院实验室迈克F810全自动血细胞分析仪报警值(原始细胞/异常淋巴细胞、核左移、异型淋巴细胞、红细胞碎片、血小板聚集、缺铁贫)至少一项大于100的标本487例进行人工显微镜镜检,收集无报警值提示的100例标本作为对照组,统计相关报警信息与镜检标准的符合率。将报警值按100~200区间和201~300区间分组,分别统计各类报警信息与镜检结果的符合率。结果 迈克F810血细胞分析仪原始细胞/异常淋巴细胞的阳性符合率为2.5%,核左移的阳性符合率为89.3%,异型淋巴细胞的阳性符合率为74.2%,红细胞碎片的阳性符合率为56.7%,血小板聚集的阳性符合率为60.4%,缺铁贫的阳性符合率为88.1%,阳性预测值最高的是核左移,阴性预测值最高的是原始细胞/异常淋巴细胞;对照组中无报警提示的标本镜检后发现真阴性97例,镜检出现2例血小板聚集,1例红细胞碎片;异型淋巴细胞、血小板聚集、缺铁贫3类报警提示与镜检结果的符合率在不同报警值区间的差异无统计学意义(P>0.05);核左移、红细胞碎片报警在不同报警值区间的阳性符合率差异有统计学意义(P<0.05)。结论 全自动血细胞分析仪原始细胞/异常淋巴细胞、核左移、异型淋巴细胞、红细胞碎片、血小板聚集、缺铁贫6项报警值提示均有较高的诊断价值,实验室可根据实际检测情况设定截断值。 Objective The relationship between the alarm value of the automatic blood cell analyzer and the microscopic examination result and its application value were investigated. Methods With the collection of 487 samples with at least one alarm value(primitive cells/abnormal lymphocytes, nuclear left shift, allotypic lymphocytes, red cell fragments,platelet aggregation, iron deficiency) greater than 100 for artificial microscopic examination, and 100 samples without alarm value prompt as the negative control group, the degree of consistency of relevant alarm information with the mirror inspection standard was counted. The alarm values were calculated according to the 100-200 interval and 201-300 interval, and the conformity of various alarm information with the microscopic inspection results was calculated. Results The positive coincidence rate of primitive cells/abnormal lymphocytes of Mike F810 blood cell analyzer was 2.5%, the positive coincidence rate of nuclear left shift was 89.3%, the positive coincidence rate of heterotypic lymphocytes was 74.2%, the positive coincidence rate of erythrocyte fragments was 56.7%, the positive coincidence rate of platelet aggregation was 60.4%, the positive coincidence rate of iron deficiency was 88.1%, the positive predictive value is the left shift of nucleus, and the negative predictive value was the original cells/abnormal lymphocytes;In the control group, 97 samples without alarm prompt were found to be true negative after microscopic examination, 2 samples with platelet aggregation and 1 sample with red cell debris were found on microscopic examination. There was no significant difference between the coincidence rate of the three types of alarm prompts of allotypic lymphocytes, platelet aggregation and iron deficiency and the microscopic examination results in different alarm value intervals(P>0.05);The positive coincidence rate of left nuclear shift and red blood cell debris alarm in different alarm value ranges was significantly different(P<0.05).
