摘要
目的 探讨直径(d)≤3cm的NSCLC发生单独N1淋巴结转移的危险因素,为术中和术后N1淋巴结清扫提供理论依据。方法 收集2013年1月至2018年12月于扬州大学临床医学院胸外科行肺癌手术共1213例患者的病例资料进行回顾性分析,选取了可能与d≤3cm的NSCLC发生单独N1淋巴结转移的30个相关危险因素,通过SPSS 22.0进行单因素、多因素Logistic回归分析。结果 单因素分析显示:年龄>50岁、有吸烟史、2cm5ng/mL,这9个因素与d≤3cm的NSCLC患者发生单独N1淋巴结转移有密切关系(P<0.05)。多因素Logistic回归分析得出:肿瘤部位(中央型)、肿瘤成分(实性成分)、毛刺征和肿瘤标志物CEA>5ng/mL这4个因素是d≤3cm的NSCLC患者发生单独N1淋巴结转移独立危险因素(P<0.05)。结论 肿瘤部位(中央型)、肿瘤成分(实性成分)、毛刺征和肿瘤标志物CEA>5ng/mL是d≤3cm的NSCLC发生单独N1淋巴结转移的独立危险因素。
Objective To explore the risk factors of single N1 lymph node metastasis in NSCLC with a diameter(d) ≤3cm, to provide a theoretical basis for intraoperative and postoperative N1 lymph node dissection. Methods A total of 1213 patients who underwent lung cancer surgery in the Department of Thoracic Surgery, Clinical School of Medicine, Yangzhou University from January 2013 to December 2018 were collected for retrospective analysis and 30 risk factors that may be associated with single N1 lymph node metastasis of NSCLC d≤3cm were selected. SPSS 22.0 was used for univariate and multivariate Logistic regression analysis.Results Univariate analysis showed that: age>50 years old, smoking history, 2cm5ng/mL, these nine factors were closely related to the occurrence of single N1 lymph node metastasis in NSCLC patients with d≤3cm(P<0.05).Multivariate Logistic regression analysis showed that: tumor site(central type), tumor component(solid component), burr sign, and tumor marker CEA>5ng/mL were independent risk factors for N1 lymph node metastasis in NSCLC patients with d≤3cm(P<0.05).Conclusion Tumor site(central type), tumor component(solid component), burr sign, and tumor marker CEA>5ng/mL are independent risk factors for the occurrence of single N1 lymph node metastasis in NSCLC with d≤3cm.
