摘要
目的 探讨不同浓度的罗哌卡因在剖宫产术后镇痛中的应用效果。方法 选取2021年1月至2022年1月于乌鲁木齐市第一人民医院行剖宫产术的产妇123例作为研究对象,采用随机数字表法分为A组、B组及C组,各41例。所有产妇术后均行经腹横筋膜平面阻滞,A组给予0.45%罗哌卡因20 ml,B组给予0.35%罗哌卡因20 ml,C组给予0.25%罗哌卡因20 ml。记录3组患者术后各时间点疼痛评分、舒适度评分、24 h镇痛泵按压次数、24 h舒芬太尼用量及不良反应发生情况。结果 A组及B组在术后各时间点的视觉模拟评分法(VAS)评分均低于C组,差异有统计学意义(P<0.05);A组及B组在术后8 h、12 h及24 h的舒适度评分(BCS)均高于C组,差异有统计学意义(P<0.05);A、B组24 h镇痛泵按压次数及舒芬太尼用量均明显少于C组,差异有统计学意义(P<0.05);A组不良反应发生率为30.95%,明显高于B组14.29%及C组11.90%,差异有统计学意义(P<0.05)。结论 0.35%罗哌卡因对剖宫产产妇具有良好的镇痛效果,同时能有效减少术后24 h舒芬太尼用量,降低不良反应发生率。
Objective To explore the application effect of different concentrations of ropivacaine in postoperative analgesia after cesarean section. Methods 123 parturients who underwent cesarean section in the First People’s Hospital of Urumqi from January 2021 to January 2022 were selected as the research objects. They were randomly divided into group A, group B and group C with 41cases each.All parturients underwent transabdominal transverse fascia plane block after operation, Group A was given 20ml of 0.45% ropivacaine, group B was given 20ml of 0.35% ropivacaine, and group C was given 20 ml of 0.25% ropivacaine. The pain score, comfort score, 24-hour analgesic pump pressing times, 24-hour sufentanil dosage and adverse reactions of the three groups were recorded. Results The VAS scores of group A and group B at each time point after operation were lower than those of group C, and the difference was significant(P<0.05);The BCS scores of group A and group B at 8 h, 12 h and 24 h after operation were significantly higher than those of group C(P<0.05);The pressing times of 24 h analgesic pump and the dosage of sufentanil in group A and group B were significantly less than those in group C(P<0.05);The incidence of adverse reactions in group A was 30.95%, which was significantly higher than 14.29% in group B and 11.90% in group C(P<0.05). Conclusion 0.35%ropivacaine had a good analgesic effect on cesarean section women, at the same time, it can effectively reduce the dosage of sufentanil 24 hours after operation and reduce the incidence of adverse reactions.
