摘要
目的:观察增液运脾汤联合基础治疗对改善慢性肾衰竭患者便秘症状的临床效果。方法:选取2020年3月-2021年8月镇江市中医院肾病科、脾胃病科及肛肠科收治的80例津亏气滞证慢性肾衰竭合并便秘患者作为研究对象,根据随机数字表法分为治疗组与对照组,各40例。两组均给予基础治疗,治疗组加用增液运脾汤治疗,对照组加乳果糖口服液口服治疗。比较两组治疗效果、便秘患者症状自评量表(PAC-SYM)评分、自发完全排便次数(SCBM)及肾功能。结果:治疗组治疗总有效率高于对照组,差异有统计学意义(P=0.037)。治疗后,治疗组粪便性状、直肠症状、腹部症状评分及PAC-SYM总分均低于对照组,差异有统计学意义(P<0.05)。治疗4周、8周后,两组SCBM均多于治疗前,且治疗组多于对照组,差异有统计学意义(P<0.05)。治疗后,两组血肌酐、尿素氮水平均低于治疗前,且治疗组低于对照组,差异有统计学意义(P<0.05)。结论:增液运脾汤联合基础治疗可明显改善津亏气滞证慢性肾衰竭患者的便秘症状,增加其排便次数,改善肾功能。
Objective:To observe the clinical effect of Zengye Yunpi Decoction combined with basic therapy for improving the chronic renal failure and constipation symptom.Methods:A total of 80 patients with chronic renal failure of syndrome of deficiency of fluid and qi stagnation complicated with constipation who were treated in Department of Nephrology,Department of SpleenStomach Diseases and Department of Proctology at Zhenjiang Hospital of Traditional Chinese Medicine from March 2020 to August2021 were selected as the study subjects.They were divided into control group and treatment group according to random number table method,with 40 cases in each group.Both groups received basic therapy.The treatment group was additionally treated with Zengye Yunpi Decoction,and the control group was additionally treated with lactulose oral solution orally.The treatment effect,patient assessment of constipation-symptoms(PAC-SYM)scale score,spontaneous complete bowel movements(SCBM)and renal function were compared between the two groups.Results:The overall effective rate of treatment in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the fecal traits,rectal symptoms,abdominal symptom scores and total PAC-SYM scores in the treatment group were lower than those in the control group,and the differences were statistically significant(P<0.05).After 4 weeks and 8 weeks of treatment,the SCBM in both groups was higher than that before treatment,and the treatment group was higher than the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of serum creatinine and urea nitrogen in the two groups were lower than those before treatment,and the treatment group was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion:The combination of Zengye Yunpi Decoction with basic therapy can significantly improve deficiency of fluid and qi stagnation constipation symptom of patients wi
