热消融治疗孕期原发性甲状旁腺功能亢进症初探

摘要 目的:探讨超声引导下经皮热消融术治疗孕期原发性甲状旁腺功能亢进症(PHPT)的临床价值。方法:选取10年间深圳市人民医院收治的6例孕期PHPT患者作为研究对象,根据治疗方法分为热消融治疗组与保守治疗组,分析两组临床特点及母儿转归进行分析。结果:热消融治疗组诊断时血钙水平均>3.0 mmol/L,热消融治疗组患者均未出术后并发症,妊娠结局良好。结论:孕期PHPT容易被延误诊治,造成不良妊娠结局。超声引导下经皮热消融术具有创伤小、无辐射、恢复期短等优势,为孕期PHPT治疗提供了新思路。 Objective:To investigate the clinical value of ultrasound-guided percutaneous thermal ablation in the treatment of primary hyperparathyroidism(PHPT)during pregnancy.Methods:From March 2011 to April 2021,a total of 6 patients with PHPT during pregnancy who were admitted to Shenzhen People’s Hospital were selected as the study subjects.According to the treatment method,the patients were divided into thermal ablation treatment group and conservative treatment group.Clinical characteristics of patients and maternal and fetal outcomes in two groups were analyzed.Results:The serum calcium level in the thermal ablation treatment group was both>3.0 mmol/L at diagnosis;No patients in the thermal ablation treatment group had postoperative complications,and their pregnancy outcome was good.Conclusion:In clinical practice,PHPT during pregnancy is easy to be misdiagnosed,resulting in adverse pregnancy outcome.Ultrasonic-guided percutaneous thermal ablation therapy has less trauma,no radiation and shorter recovery period,it may provide new ideas for the treatment of PHPT during pregnancy.
