摘要
目的探究杭州地区血红蛋白(Hb)变异体的主要类型及其对毛细管电泳法和高效液相色谱法糖化血红蛋白(HnA1c)检测系统的影响情况。方法收集2021年1—8月浙江大学医学院附属邵逸夫医院钱塘院区住院、门诊患者以及健康体检者血液样本5068份,分别检测受试者Hb、平均红细胞体积(MCV)、平均Hb浓度(MCH)、空腹血糖(FPG)和毛细管电泳法HbA1c,并对仪器提示的变异体样本进行Hb电泳和基因测序,确认变异体类型。Hb变异体样本进行高效液相色谱法HbA1c复检,统计分析其差异性。结果5068份血液样本中共检出变异体20份,占总数的0.39%,变异体包括10份HbG-Taipei,3份HbG-Honolulu,2份HbE,2份HbQ-Thailand,1份HbG-Coushatta,1份HbUbe-2和1份HbNew York。10份HbG-Taipei变异体中,高效液相色谱法检测HbA1c偏倚率均>20%,其中5份HbA1c异常误判为正常,且该方法对HbA1c的检测结果为(4.66±0.86)%,低于毛细管电泳法的(6.19±0.88)%,差异有统计学意义(t=-9.37,P<0.001)。结论HbG-Taipei为杭州地区最流行的Hb变异体,对该变异体HbA1c的检测,毛细管电泳法较高效液相色谱法的结果更加真实可靠。
Objective To explore the main types of hemoglobin(Hb)variants in Hangzhou and the their influence on capillary electrophoresis and high performance liquid chromatography on glycated hemoglobin(HbA1c)detection system.Methods A total of 5068 serum samples were collected from inpatients,outpatients and health examiners in Qiantang Section of Sir Run Run Shaw Hospital,Zhejiang University School of Medicine from January to August 2021.Levels of Hb,mean erythrocyte volume(MCV),mean erythrocyte Hb(MCH),fasting blood glucose(FPG)and HbA1c by capillary electrophoresis were detected.Hb electrophoresis and gene sequencing were performed on the variant samples suggested by the instrument.Hb variant samples were re-examined for HbA1c by high performance liquid chromatography,and their differences were statistically analyzed.Results A total of 20 variants were detected in 5068 samples,accounting for 0.39%.These variants included 10 HbG-Taipei variants,3 HbG-Honolulu variants,2 HbE variants,2 HbQ-Thailand variants,1 HbG-Coushatta variant,1 HbUbe-2 variant and 1 HbNew York variant.The bias rates of HbA1c detected by high performance liquid chromatography were all>20%in 10 HbG-Taipei variants,and 5 variants were misdiagnosed as normal.There was statistically different between capillary electrophoresis[(6.19±0.88)%]and high performance liquid chromatography[(4.66±0.86)%](t=-9.37,P<0.001).Conclusions HbG-Taipei is the most popular Hb variant in Hangzhou area,and the detection results of HbA1c by capillary electrophoresis are more reliable than those by high performance liquid chromatography.
