摘要
目的^(18)F-脱氧葡萄糖(^(18)F-FDG)正电子发射断层显像(PET/CT)和钼靶摄影对乳腺癌及腋窝淋巴结(ALN)转移的诊断准确性.方法选取2013年9月至2020年11月收治的70例病理证实乳腺癌患者,均行^(18)F-FDG PET/CT和钼靶摄影,测量乳腺癌病灶及ALN的最大标准摄取值(SUVmax),并进行乳腺影像报告与数据系统(BI-RADS)分类.比较^(18)F-FDG PET/CT和钼靶摄影诊断乳腺癌及ALN转移的诊断效能.结果乳腺癌病灶的SUVmax为(10.36±5.93).转移性ALN的SUVmax为(9.43±4.73),未转移性ALN的SUVmax为(2.08±0.25),差异有统计学意义(P<0.05).^(18)F-FDG PET/CT和钼靶摄影对乳腺癌的灵敏度分别为98.6%和83.3%,差异无统计学意义(P>0.05);对ALN转移的灵敏度、特异度、准确度分别为100%、81.8%、95.1%和46.7%、90.9%,58.5%,差异有统计学意义(P<0.05).PET/CT还发现32例有远处病灶.结论^(18)F-FDG PET/CT和钼靶摄影均是诊断乳腺癌的有效方法.但PET/CT对腋窝淋巴转移具有更好的诊断效能,且在探查远处转移病灶更具有优势.
Objective To compare the diagnostic accuracy between ^(18)F-FDG PET/CT and mammography in diagnosis of breast cancer and aillary lymph node metastasis.Methods ^(18)F-FDG PET/CT and mammography were performed on 70 patients who were pathologically confirmed as breast cancer from September 2013 to November 2020 were selected.The SUVmax of breast lesions were obtained.Mammographic classification of breast lesions was obtained with breast imaging reporting and data system(BI-RADS),The diagnostic accuracy between ^(18)F-FDG PET/CT and mammography in diagnosis of breast cancer and axillary lymph node metastasis were compared.Results The SUVmax of breast lesions was 10.36±5.93.The SUVmax of malignant ALN lesions was 9.43±4.73,of benign ALN lesions was 2.08±0.25(P<0.05).The sensitivity value obtained by ^(18)F-FDG PET/CT for malignant breast lesions was 98.6% respectively,while 83.3% for mammography(P>0.05).The sensitivity,specificity and accuracy value obtained by ^(18)F-FDG PET/CT for ALN lesions were 100%,81.8% and 95.1% respectively,while 46.1%,90.9% and 58.5% for mammography(P<0.05).PET/CT found that 32 patient had metastatic lesions.Conclusion The ^(18)F-FDG PET/CT and mammography are both effective approaches to the diagnosis of breast cancer ^(18)F-FDG PET/CT has better ficacy in diagnosis of benign and malignant ALN lesions,and has more advantageous in exploringdistantlesions.
