摘要
目的探讨孕晚期宫颈弹性成像参数与第一产程潜伏期、活跃期时间的相关性.方法回顾性收集2021年04月至2021年12月本院67例足月待产孕妇的临床资料,经阴道超声评估宫颈位置、宫颈内口形态、宫颈长度(CL),测量出宫颈弹性成像参数,包括弹性对比指数(ECI)、硬度比(HR)、宫颈内口应变平均值(IOS)、宫颈外口应变平均值(EOS)以及IOS/EOS比值,分别记录临产后第一产程时间,根据第一产程时间分为第一产程正常组与第一产程延长组;同时将顺利进行阴道分娩者的第一产程时间分成潜伏期与活跃期两组.分析孕晚期宫颈弹性成像参数与生产第一产程时间及其潜伏期、活跃期时间的相关性.结果67例临产孕妇57例经阴道分娩,其宫颈弹性成像参数提示第一产程总时间与ECI呈负相关(r=-0.403,P<0.05);第一产程潜伏期时间与ECI呈负相关(r=-0.419,P<0.05);第一产程活跃期时间与IOS/EOS呈负相关(r=-0.448,P<0.05).结论宫颈弹性成像可通过数据来客观评价宫颈成熟度情况,同时对预估产程进展有一定指导价值.
Objective To investigate the correlation between cervical elastography parameters in the third trimester of pregnancy and the latency and the active phase of the first stage of labor.Methods The clinical data of 67 expectant women from April 2021 to December 2021 were retrospectively collected and analyzed.The cervical position,shape of cervical internal opening and cervical length(CL)were evaluated by transvaginal ultrasound.The parameters of cervical elastography were measured,including elasticity contrast index(ECI),hardness ratio(HR),mean strain of cervical internal orifice(IOS),mean strain of cervical external orifice(EOS)and IOS/Eos ratio.The time of the first stage of labor was recorded and divided into two groups according to the time of the first stage of labor:the group with normal first stage of labor and the group with prolonged first stage of labor.At the same time,the first stage of labor was divided into two groups:latent period and active period.The correlation between cervical elastography parameters in the third trimester of pregnancy and the time of the first stage of labor was analyzed.Results Among 67 parturient women,57 of them had vaginal delivery.The parameters of cervical elastography showed that the total time of the first stage of labor was negatively correlated with ECI(R=-0.403,P<0.05).The latency of the first stage of labor was negatively correlated with ECI(R=-0.419,P<0.05),and the activity of the first stage of labor was negatively correlated with IOS/Eos(R=-0.448,P<0.05).Conclusion Cervical elastography can be used to evaluate the cervical maturity objectively and to predict the progress of labor.
