摘要
目的比较微创拔牙与锤凿劈冠法治疗下颌低位阻生智齿患者的效果与安全性。方法选取2020年1月至12月我院收治的90例下颌低位阻生智齿患者作为研究对象,以拔牙治疗方案将其分为常规组与观察组,各45例。常规组采取锤凿劈冠法治疗,观察组采取微创拔牙法治疗。比较两组的治疗效果。结果术后,观察组的抗牙龈卟啉单胞菌的IgG抗体(PgIgG)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)水平低于常规组(P<0.05)。观察组的术中并发症(舌侧骨板骨折、根折、邻牙松动)及术后并发症(张口受限、面颊肿胀、干槽症)发生率均低于常规组(P<0.05)。观察组术后3、5 d的视觉模拟评分法(VAS)评分低于常规组,术后7、30 d的牙槽骨密度高于常规组(P<0.05)。结论相较于锤凿劈冠法,微创拔牙治疗下颌低位阻生智齿患者的效果更好,具有更高的应用价值。
Objective To compare the efficacy and safety of minimally invasive tooth extraction and hammer-chisel split crown method in the treatment of patients with mandibular low impacted wisdom teeth.Methods A total of 90patients with mandibular low impacted wisdom teeth admitted in our hospital from January to December 2020 were selected as research objects and divided into conventional group and observation group by means of tooth extraction treatment,with 45 cases in each group.The conventional group was treated with hammer-chisel split crown method,and the observation group was treated with minimally invasive tooth extraction.The therapeutic effects of the two groups were compared.Results After surgery,the Porghyromonas gingivalis IgG antibody(PgIgG),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)levels in the observation group were lower than those in the conventional group(P<0.05).The incidence of intraoperative complications(lingual plate fracture,root fracture,adjacent tooth loosening)and postoperative complications(restricted mouth opening,cheek swelling,dry groove)in the observation group were lower than those in the conventional group(P<0.05).The Visual Analogue Scale(VAS)score at 3rd and 5th day after surgery in the observation group was lower than that in the conventional group,and alveolar bone density at 7th and 30th day after surgery in the observation group was higher than that in the conventional group(P<0.05).Conclusion Compared with the hammer-chisel split crown method,minimally invasive tooth extraction is more effective in the treatment of mandibular low impacted wisdom teeth patients,and has higher application value.
