摘要
目的 探讨内镜下行鼓室置管术对鼻咽癌放疗后分泌性中耳炎(OME)患者术后疼痛、听力及炎症反应的影响。方法 选取本院2020年1月至2021年1月收治的鼻咽癌放疗后OME患者90例,按照随机数字表法分为两组,每组45例。对照组采用鼓室注药治疗,观察组采用内镜下鼓膜置管术治疗。比较两组疗效、视觉模拟疼痛评分(VAS)、纯音听阈值、炎症因子[白介素(IL)-1β、IL-5、转化生长因子β1(TGF-β1)]及并发症。结果 观察组总有效率(93.33%)高于对照组(73.33%)(P=0.011)。术后6个月,两组VAS评分、纯音听阈值均降低,且观察组低于对照组(P<0.001)。术后6个月,两组血清IL-1β、IL-5和TGF-β1水平均降低,且观察组均低于对照组(P<0.001)。观察组并发症率(15.55%)低于对照组(35.55%)(P=0.030)。结论 内镜下行鼓室置管术治疗鼻咽癌放疗后OME效果显著,能减轻患者术后疼痛,改善听力,降低血清IL-1β、IL-5、TGF-β1水平,减轻炎症反应,且并发症少,安全性较高。
Objective To investigate the effect of endoscopic tympanic membrane catheterization on postoperative pain, hearing and inflammatory response in patients with otitis media with effusion(OME) after radiotherapy for nasopharyngeal carcinoma. Methods 90 patients with OME after radiotherapy for nasopharyngeal carcinoma in our hospital from January 2020 to January 2021 were randomly divided into two groups, with 45 cases in each group. The control group was injected with medicine in tympanic cavity, and the observation group was treated with endoscopic tympanic membrane catheterization.The efficacy, visual analogue scale(VAS), pure tone hearing threshold, inflammatory factors [interleukin(IL)-1β, IL-5, transforming growth factor β1(TGF-β1)] and complications were compared. Results The total effective rate(93.33%) of the observation group was higher than that of the control group(73.33%)(P=0.011). Six months after operation, the VAS score and pure tone hearing threshold of the two groups decreased, and the observation group was lower than that of the control group(P<0.001).Six months after operation, the serum levels of IL-1β, IL-5 and TGF-β1 in both groups were decreased, and the observation group was lower than the control group(P<0.001).The complication rate of the observation group(15.55%) was lower than that of the control group(35.55%)(P=0.030). Conclusion Endoscopic tympanic catheterization is effective in the treatment of OME after radiotherapy for nasopharyngeal carcinoma. It can reduce postoperative pain, improve hearing and reduce serum level of IL-1β, IL-5, TGF-β1, reduce inflammation, with less complications and high safety.
