摘要
目的 探讨自固定补片在腹腔镜完全腹膜外疝修补术(TEP)中应用的临床疗效。方法 选取北部战区总医院普通外科自2018年6月至2021年11月收治的52例腹股沟疝患者为研究对象,所有患者行TEP治疗,并在术中应用自固定补片。记录并分析患者手术时间、术中出血量、术后视觉模拟评分法(VAS)评分、术后平均住院时间以及不良反应发生情况等。结果 本组52例患者均成功完成手术,单侧疝手术时间(47.0±7.3)min,双侧疝手术时间(69.0±7.4)min,术中出血量少,VAS评分(2.0±0.7)分,无急性剧烈疼痛(无VAS评分>4分患者),术后平均住院时间(1.4±0.7) d。术后出现皮下气肿1例,未经处理自行吸收;血清肿2例,均自行吸收;所有患者均未发生尿潴留。本组52例患者均获得13~54个月的随访,末次随访时均无再次复发、无慢性持续性疼痛、无局部异物感、无补片感染、无切口感染。结论 TEP术中应用自固定补片,安全、有效,且可较少并发症的发生情况。
Objective To investigate the clinical effect of self-gripping mesh in laparoscopic totally extraperitoneal(TEP).Methods Fifty-two patients with inguinal hernia admitted from June 2018 to November 2021 were selected as the research objects.All patients were treated with TEP and self-gripping meshes were used during the operation.The operation time,blood loss,visual analogue scale(VAS),average hospital stay and adverse reactions were recorded and analyzed.Results The operation time of unilateral hernia was(47.0±7.3)min, bilateral hernia was(69.0±7.4)min,the amount of bleeding was less,the VAS score was(2.0±0.7),there was no acute severe pain(no patients with VAS>4),and the average postoperative hospital stay was(1.4±0.7)d.Subcutaneous emphysema occurred in 1 case after operation,which was absorbed by itself without treatment.Serum swelling occurred in 2 cases,all of which were absorbed by themselves.No urinary retention occurred in all patients.All 52 patients were followed up for 13~54 months.At the last follow-up,there was no recurrence,chronic persistent pain,local foreign body sensation,mesh infection and incision infection.Conclusion The application of self-gripping mesh in TEP is safe and effective,and can reduce the incidence of complications.
