血清SF、hs-CRP和25-(OH)D_(3)联合检测对妊娠期糖尿病的早期预测研究 被引量:1

摘要 目的探讨血清铁蛋白(SF)、超敏-C反应蛋白(hs-CRP)和25-羟基维生素D_(3)[25-(OH)D_(3)]联合检测对妊娠期糖尿病(GDM)的早期预测价值。方法选取2021年8月至2022年7月在中国科学院大学深圳医院就诊建档的300名妊娠8~13周单胎妊娠孕妇作为研究对象,根据妊娠24~28周期间的口服葡萄糖耐量试验(OGTT)结果,将孕妇分为GDM组(n=71)和非GDM组(n=229)。比较两组孕妇妊娠8~13周时的血清SF、hs-CRP和25-(OH)D_(3)水平差异,分析三者单一与联合检查预测GDM的价值,比较两组孕妇不良妊娠结局发生情况。结果GDM组孕妇的血清SF、hs-CRP高于非GDM组,25-(OH)D_(3)水平低于非GDM组,差异有统计学意义(P<0.05);SF、hs-CRP和25-(OH)D_(3)联合诊断预测GDM的曲线下面积(AUC)为0.909,均高于单一检查(P<0.05)。结论GDM患者妊娠早期的血清SF、hs-CRP明显升高,而25-(OH)D_(3)水平下降,三者联合检查能够提高GDM的早期预测价值。 Objective To explore the early predictive value of combined detection of serum ferritin(SF),hypersensitive C-reactive protein(hs-CRP)and 25 hydroxyvitamin D_(3)(25-[OH]D_(3))in gestational diabetes mellitus(GDM).Methods A total of 300 pregnant women with singleton pregnancy of 8 to 13 weeks who were treated and filed in Shenzhen Hospital of the University of Chinese Academy of Sciences from August 2021 to July 2022 were selected as subjects.According to the oral glucose tolerance test(OGTT)results during 24 to 28 weeks of pregnancy,pregnant women were divided into GDM group(n=71)and non GDM group(n=229).The difference of serum SF,hs-CRP and 25-(OH)D_(3)levels between the two groups at 8 to 13 weeks gestation was compared,the value of single and combined tests of the three tests in predicting GDM was analyzed,and the occurrence of adverse pregnancy outcomes between the two groups were compared.Results The serum SF and hs-CRP levels in GDM group were higher than those in non-GDM group,and the 25-(OH)D_(3)level in GDM group was lower than that in non-GDM group,and the differences were statistically significant(P<0.05).The combined diagnosis of SF,hs-CRP and 25-(OH)D_(3)predicted GDM with an area under the curve(AUC)of 0.909,which was higher than that of single test(P<0.05).Conclusion Serum SF and hs-CRP in patients with GDM in early pregnancy are significantly up-regulated,while the level of 25-(OH)D_(3)is decreased.The combined examination of the three can improve the early predictive value of GDM.
