关节镜下肘关节松解术治疗肘关节屈伸功能障碍早期临床疗效分析 被引量:2

摘要 目的探究关节镜下肘关节松解术治疗肘关节屈伸功能障碍的早期临床疗效。方法回顾性分析2020年3月至2021年12月在西南医科大学附属医院骨与关节外科采用关节镜下肘关节松解术治疗肘关节屈伸功能障碍患者的临床资料,共纳入23例,其中男19例,女4例;年龄22~62岁,平均(44.3±11.3)岁。所有患者均采用关节镜下肘关节松解术治疗,对存在关节腔游离体的患者将游离体予以取出,对伴有尺神经炎的患者予以尺神经松解前置术。记录术前、术后即刻、术后1个月、术后6个月肘关节最大屈伸角度,以及术前与术后1个月、术后6个月患者疼痛视觉模拟评分(VAS)、Mayo肘关节功能评分(MEPS)、美国特种外科医院(HSS)肘关节功能评分。结果所有患者肘关节最大屈伸角度均有明显改善,术前最大伸直角度和最大屈曲角度与术后即刻、术后1个月、术后6个月比较,差异均有统计学意义(P<0.05)。术前VAS评分、MEPS评分、HSS肘关节功能评分与术后1个月、术后6个月比较,差异均有统计学意义(P<0.05)。结论关节镜下肘关节松解术治疗肘关节屈伸功能障碍具有较好的早期临床疗效,可明显改善肘关节屈伸功能,具有切口小、恢复快、神经损伤风险低等优势。 Objective To investigate the early clinical efficacy of arthroscopic arthrolysis for elbow flexion and extension dysfunction.Methods From March 2020 to December 2021,a total of 23 patients treated with arthroscopic arthrolysis surgery in the Department of Bone and Joint Surgery of the Affiliated Hospital of Southwest Medical University obtained complete follow-up,of whom 19 were males and 4 were females.The age of the patients ranged from 22 to 62,with an average age of(44.3±11.3).All patients were treated with elbow arthroscopic arthrolysis,removing loose bodies for those who had them in the joint cavity and applying decompression and anterior transposition of the ulnar nerve for those with ulnar neuritis.The maximum flexion and extension angles of the elbow joint were recorded before surgery,immediately after surgery,one month after surgery,and six months after surgery.The visual analogue scale(VAS)score,Mayo elbow performance score(MEPS),and elbow function score[Hospital of Special Surgery(HSS)]were also recorded before surgery,one month after surgery,and six months after surgery.Statistical analysis was performed after the data were collected.Results The maximum flexion and extension angles of the elbow joint were significantly improved in all patients.The maximum flexion and extension angles of the elbow joint immediately after surgery,one month after surgery,and six months after surgery were significantly improved compared with those before surgery(P<0.05).The VAS,MEPS,and HSS scores one month and six months after surgery were also significantly improved compared with those before surgery(P<0.05).Conclusion Arthroscopic arthrolysis treatment of elbow flexion and extension dysfunction had a good early clinical effect,with a small incision,quick recovery,and low risk of nerve injury.
