2型糖尿病合并周围神经病变患者临床特点与神经电生理检测结果的相关性分析 被引量:3

Correlation Analysis of Clinical Characteristics and Neurophysiological Test Results in Patients with Type 2 Diabetes Mellitus Combined with Peripheral Neuropathy

摘要 目的分析2型糖尿病(type 2 diabetes mellitus,T2DM)合并周围神经病变患者临床特点与神经电生理检测结果的相关性。方法选取2020年5月—2022年5月建瓯市立医院收治的98例T2DM患者为研究对象,对所有患者施行神经电生理检测,以是否合并周围神经病变分组,对比两组空腹血糖(FPG)、糖化血红蛋白(HbA1c)、病程之间的差异;另对比临床特点内有差异项目患者的感觉神经传导速度(SCV)差异;此外以Pearson相关性分析患者临床特点与神经电生理检测结果的相关性。结果98例T2DM患者中共诊断出62例周围神经病变,T2DM合并周围神经病变患者的病程长于T2DM未合并周围神经病变患者,差异有统计学意义(P<0.05);病程≥11年的T2DM合并周围神经病变患者的正中神经、尺神经、腓肠神经、腓总神经的SCV低于病程<11年T2DM未合并周围神经病变患者,差异有统计学意义(P<0.05);Pearson相关性分析显示:患者病程与正中神经、尺神经、腓肠神经、腓总神经的SCV呈负相关(r=0.428、0401、0434、0502,P<0.05)。结论T2DM合并周围神经病变患者的各神经SCV较慢,且患者病程越长,则各神经SCV越慢。 Objective To analyze the correlation between clinical characteristics and neurophysiological test results in patients with type 2 diabetes mellitus(T2DM)combined with peripheral neuropathy.Methods Selected 98 T2DM patients admitted to Jian’ou Municipal Hospital from May 2020 to May 2022 as the study object,performing neuroelectrophysiological testing for all patients with peripheral neuropathy,comparing the differences between fasting blood glucose(FPG),glycated hemoglobin(HbA 1 c)and the duration of the disease;comparing the sensory nerve conduction velocity(SCV)of the patients with different clinical characteristics;in addition,the correlation between the clinical characteristics of patients and the neuroelectrophysiological testing results was analyzed by Pearson correlation.Results A total of 62 cases of peripheral neuropathy were diagnosed in 98 T2DM patients.The duration of T2DM patients with peripheral neuropathy was longer than that of T2DM patients without peripheral neuropathy,and the difference was statistically significant(P<0.05).The SCV of median nerve,ulnar nerve,sural nerve and common peroneal nerve in patients with T2DM and peripheral neuropathy with a course of≥11 years was lower than that in patients with T2DM and no peripheral neuropathy with a course of<11 years,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that the course of the disease was negatively correlated with the SCV of median nerve,ulnar nerve,sural nerve and common peroneal nerve(r=0.428,0401,0434,0502,P<0.05).Conclusion The SCV of each nerve was slower in patients with T2DM combined with peripheral neuropathy,and the longer the patients’disease duration,the slower the SCV of each nerve.
