压力应变环参数评估类风湿关节炎患者的心肌做功

Pressure-strain Loop Parameters for the Evaluation of Myocardial Work in Patients with Rheumatoid Arthritis

摘要 目的:应用压力应变环(PSL)参数评估类风湿关节炎(RA)患者左心室做功状况.方法:前瞻性选取扬州大学附属医院2021年5月-2022年2月确诊的RA患者90例,按病程≤1年、1~5年和>5年依次分为A组、B组和C组,每组30例,另以同期30名健康志愿者为对照组.对比分析各组传统超声心动图参数、整体心肌纵向应变(GLS)、左室整体心肌做功指数(LVGWI)、左室整体有用功(LVGCW)、左室整体无用功(LVGWW)和左室整体做功效率(LVGWE).结果:与对照组比较,A组LVGWW增加,LVGWE下降;B组和C组GLS绝对值、LVGWI、LVGCW、LVGWE明显下降,LVGWW明显升高,且差异均有统计学意义(均P<0.05).GLS绝对值、LVGWI、LVGCW、LVGWE按A组、B组、C组顺序逐渐降低,LVGWW按顺序逐渐增高,差异均有统计学意义(均P<0.05).GLS、LVGWI、LVGCW、LVGWW、LVGWE的组内相关系数(ICC)在医师内分别为0.947、0.959、0.911、0.957、0.904,医师间分别为0.906、0.953、0.908、0.922、0.871,医师内比医师间高,复测信度高.受试者工作特征曲线分析显示GLS、LVGWW的曲线下面积分别为0.858、0.957,可准确地用于RA患者左心室做功降低的诊断.结论:PSL能有效评估RA患者左心室心肌做功状况,为临床的诊断评估提供新的检测技术. Purpose:To evaluate left ventricular myocardial work of patients with rheumatoid arthritis(RA)by using pressure strain loop(PSL)parameters.Methods:Prospectively,90 patients with a definitive diagnosis of RA in the Affiliated Hospital of Yangzhou University from May 2021 to February 2022 were selected and divided into group A(n=30),group B(n=30)and group C(n=30)according to the course of disease≤1 year,1-5 years and>5 years.In addition,30 healthy volunteers were recruited as the control group in the same period.The parameters of traditional echocardiography,global longitudinal strain(GLS),left ventricle global work index(LVGWI),left ventricle global constructive work(LVGCW),left ventricle global waste work(LVGWW)and left ventricle global work efficiency(LVGWE)were compared and analyzed.Results:Compared with the control group,LVGWW was increased and LVGWE was decreased in Group A.GLS absolute value,LVGWI,LVGCW and LVGWE decreased significantly and LVGWW increased significantly in Group B and C,and the differences were statistically significant(all P<0.05).GLS absolute value,LVGWI,LVGCW,LVGWE gradually decreased and LVGWW increased in the order of group A,group B,and group C,and the differences were statistically significant(all P<0.05).The interclass correlation coefficients(ICC)of GLS,LVGWI,LVGCW,LVGWW,and LVGWE were 0.947,0.959,0.911,0.957,
