不同类型肠道病毒感染致小儿手足口病的临床特点分析 被引量:2

摘要 目的比较不同类型肠道病毒感染致小儿手足口病(HFMD)的临床特点,为小儿HFMD有效防治提供理论依据。方法收集南阳市第一人民医院感染科2019年1月至2021年6月收治的150例HFMD患儿的临床资料,采用实时荧光定量聚合酶链式反应技术检测HFMD患儿标本中肠道病毒感染情况,将肠道病毒阳性的148例患儿分为EV71组(98例)、CoxA16组(45例)和其他肠道病毒组(5例)。比较三组HFMD患儿的发病特点、实验室指标和预后情况。结果EV71组口腔疱疹、抽搐发生率、最高体温高于CoxA16组和其他肠道病毒组,发热天数长于CoxA16组和其他肠道病毒组(P<0.05)。EV71组白细胞计数、降钙素原、C反应蛋白、乳酸脱氢酶、肌酸激酶同工酶水平高于CoxA16组和其他肠道病毒组(P<0.05),而CD3+CD4+、CD3+CD8+低于CoxA16组和其他肠道病毒组(P<0.05)。EV71组呼吸系统并发症、中枢神经系统并发症发生率高于CoxA16组、其他肠道病毒组,但差异未见统计学意义(P>0.05);EV71组痊愈、死亡率与CoxA16组、其他肠道病毒组比较差异未见统计学意义(P>0.05)。结论不同类型肠道病毒感染致HFMD的年龄和性别分布特征基本相似,且预后相当。临床应加强不同类型肠道病毒感染致HFMD的临床特点和实验室指标的分析,为小儿HFMD的临床防治提供依据。 Objective To compare the clinical features of hand-foot-mouth disease(HFMD)induced by different types of enterovirus infection in children,and provide theoretical basis for effective prevention against HFMD.Methods The clinical data of 150 HFMD children who were admitted to Department of Infectious Disease of Nanyang First People’s Hospital from January 2019 to June 2021 were collected.The enterovirus infection situations of HFMD children were detected by real-time fluorescence quantitative polymerase chain reaction,and 148 children with positive enterovirus were divided into EV71 group(98 cases),CoxA16 group(45 cases)and other enterovirus group(5 cases).The onset features,laboratory indexes and prognosis were compared among the three group.Results The incidence of oral herpes and convulsions,and maximum temperature in the EV71 group were significantly higher than those in the CoxA16 group and other enterovirus group,and fever duration was significantly longer than that in the CoxA16 group and other enterovirus group(P<0.05).The levels of white blood cell count,procalcitonin,C-reactive protein,lactate dehydrogenase and creatine kinase isoenzyme in the EV71 group were significantly higher than those in the CoxA16 group and other enterovirus groups(P<0.05),while cluster of differentiation 3+(CD3+)plus cluster of differentiation 4+,and CD3+plus cluster of differentiation 8+were significantly lower than those in the CoxA16 group and other enterovirus group(P<0.05).The incidence of respiratory system and central nervous system complications in the EV71 group were higher than that in the CoxA16 group and other enterovirus group,but the differences were not statistically significant(P>0.05).There was no significant difference in cure rate and case fatality rate among the EV71 group,CoxA16,and other enterovirus group(P>0.05).Conclusions The distributions of age and gender are basically similar among children with HFMD induced by different types of enterovirus infection,and they have similar prognoses.The analysis on cl
