摘要
目的:探讨妊娠期糖尿病(GDM)孕妇孕中期糖脂代谢指标与大于胎龄儿(LGA)的相关性及其预测价值。方法:选取2021年2月1日至2022年1月31日于深圳市宝安区妇幼保健院产检并分娩的GDM孕妇2114例。按新生儿出生体重分为非LGA组(1885例)和LGA组(229例)。回顾分析孕妇的一般情况和孕中期糖脂代谢指标,探讨其与LGA的相关性及预测价值。结果:2114例GDM孕妇中分娩LGA新生儿229例,发生率10.83%。LGA组的孕妇年龄、孕前BMI、孕次、产次、巨大儿史发生率均高于非LGA组,差异有统计学意义(P<0.001)。LGA组和非LGA组的空腹血糖、甘油三脂、糖化血红蛋白比较,差异有统计学意义(P<0.001)。Logistic回归分析显示,孕前BMI、产次、巨大儿分娩史、空腹血糖、甘油三酯是发生LGA的独立危险因素(OR>1,P<0.05)。孕中期空腹血糖及甘油三酯预测LGA的AUC分别为0.624和0.597,差异有统计学意义(P<0.05)。结论:孕中期空腹血糖及甘油三脂与LGA发生相关,但单一指标对LGA的预测意义价值不高。
Objective:To investigate the correlation between glucose and lipid metabolism indexes in the second trimester of pregnant women with gestational diabetes mellitus and large for gestational age infants.Methods:Enrolled 2114 pregnant women who were diagnosed as gestational diabetes mellitus(GDM)in Shenzhen Baoan Women's and Children's Hospital from February 1,2021 to January 31,2022.The patients were divided into non-large for gestational age(1885 cases)and large for gestational age group(229 cases)according to the birth weight of their newborns.The general condition of pregnant women and the glucose and lipid metabolism indexes in the second trimester were retrospectively analyzed to explore their correlation with large for gestational age infants and their predictive value.Results:Among 2114 pregnant women with gestational diabetes mellitus(GDM),229 cases delivered LGA neonates,the incidence rate was 10.83%.The age,gravidity,parity and pre-pregnancy BMI of the pregnant women with large gestational age group was higher than that of the non-large gestational age group,and the difference was statistically significant(P<0.001).The FPG and triglyceride level in the large for gestational age group was significantly higher than that in the non-large for gestational age group(P<0.001).Logistic regression analysis showed that pre-pregnancy BMI、GDM history、FBG、and triglyceride was an independent risk factor for large for gestational age(OR>1,P<0.05).The AUC of fasting blood glucose and triglyceride in the second trimester were 0.624 and 0.597(P<0.05).Conclusion:FBG and triglyceride in the second trimester are associated with the occurrence of large for gestational age infants,but the value of single index in predicting large for gestational age infants is not high.
