摘要
目的:探讨经尿道钬激光前列腺剜除术对前列腺增生(BPH)患者术后排尿功能及临床症状评分的影响。方法:选取2019年1月-2022年6月枣庄市山亭区人民医院收治的131例BPH患者,按照患者接受手术方式的异同将其分组,将58例接受经尿道钬激光前列腺剜除术治疗的患者划分为观察组,将73例接受经尿道前列腺等离子电切术治疗的患者划分为对照组,对比两组患者的手术相关指标、国际前列腺症状评分(IPSS)、生活质量评分(Qo L);平均尿流率(Q_(ave))、最大尿流率(Q_(max))及残余尿量(PVR);并发症发生率。结果:观察组的手术时间、术后留置导管时间、膀胱冲洗时间、术后住院时间均短于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05)。术后,观察组的IPSS评分、Qo L评分、PVR及并发症发生率均低于对照组,Q_(ave)及Q_(max)均高于对照组,差异均有统计学意义(P<0.05)。结论:经尿道钬激光前列腺剜除术治疗BPH,手术时间、术后留置导管时间及住院时间等均较短,利于患者排尿功能及生活质量的改善,术后并发症发生率低,安全性较高,整体效果优异。
Objective:To investigate the effects of transurethral holmium laser prostate enucleation on postoperative voiding function and clinical symptom scores in patients with benign prostatic hyperplasia(BPH).Method:A total of 131 patients with BPH admitted to Zaozhuang Shanting District People’s Hospital from January2019 to June 2022 were selected.They were divided into two groups according to the similarities and differences of the surgical methods.58 patients treated with transurethral holmium laser prostate enucleation were classified as the observation group and 73 patients treated with transurethral prostate plasma electrosurgery were classified as the control group,the operation related indexes,international prostate symptom score(IPSS),quality of life score(Qo L),mean urinary flow rate(Q_(ave)),maximum urinary flow rate(Q_(max)),residual urine volume(PVR)and incidence of complications of the two groups were compared.Result:The operative time,postoperative indwelling catheter time,bladder flushing time,postoperative hospital stay were shorter than those in the control group,and intraoperative bleeding in the observation group was less than that in the control group,the differences were statistically significant(P<0.05).After surgery,the IPSS score,Qo L score,PVR and complication rate of the observation group were lower than those of the control group,and Q_(ave)and Q_(max)were higher than those of the control group,with statistically significant differences(P<0.05).Conclusion:Transurethral holmium laser prostate enucleation for BPH has shorter operative time,postoperative indwelling catheter time and hospital stay,facilitates the improvement of patients’voiding function and quality of life,has low postoperative complication rate,has higher safety,and has excellent overall results.
