摘要
目的 探讨不同剂量盐酸羟考酮对腹腔镜子宫切除术患者的应用效果。方法 回顾性收集2019年5月至2021年12月于金华市人民医院行腹腔镜子宫切除术的62例患者的病历资料,根据患者术后镇痛使用的盐酸羟考酮剂量分为Ⅰ组(n=30)和Ⅱ组(n=32),Ⅰ组剂量为0.1mg/kg,Ⅱ组剂量为0.15mg/kg。比较两组术后疼痛应激反应、氧化应激反应、炎症、不良反应发生情况。结果 Ⅱ组术后4h、8h、12h以及24h时视觉模拟评分(visual analogue scale,VAS)均低于Ⅰ组,差异有统计学意义(P<0.05);术后12h时,两组血糖(blood glucose,Glu)、去甲肾上腺素(norepinephrine,NE)、皮质醇(cortisol,Cor)水平均上升,但Ⅱ组Glu、NE、Cor水平均低于Ⅰ组,差异有统计学意义(P<0.05);术后24h,两组白细胞介素(interleukin,IL)-6、IL-10及肿瘤坏死因子-α(tumor necrosis factor,TNF-α)水平均上升,但Ⅱ组IL-6、IL-10及TNF-α水平低于Ⅰ组,差异有统计学意义(P<0.05);两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 相较于0.1mg/kg剂量,0.15mg/kg剂量的盐酸羟考酮更有利于缓解腹腔镜子宫切除术患者术后疼痛程度,减轻术后应激反应和炎症,且不增加药物不良反应发生风险。
Objective To investigate the effccts of different doses of oxycodone hydrochloride on laparoscopic hysterectomy.Methods The medical records of 62 patients who underwent laparoscopic hysterectomy in Jinhua Municipal People’s Hospital from May 2019 to December 2021 were retrospectively collected and divided into group Ⅰ(n=30) and group Ⅱ(n=32)according to the dose of oxycodone hydrochloride used after operation. The doses of oxycodone hydrochloride in group Ⅰ and group Ⅱ were 0.1mg/kg and 0.15mg/kg. Postoperative pain stress response, oxidative stress response, inflammatory response and adverse reactions were compared between the groups. Results Group Ⅱ had lower visual analogue scale(VAS) than group Ⅰ at 4h, 8h, 12h and 24h after operation(P<0.05). The levels of GLU, NE and Cor in the two groups increased at 12h after operation, and were lower in group Ⅱ than those in group Ⅰ, and the differences were statistically significant(P<0.05).The levels of(interleukin, IL)-6, IL-10 and tumor necrosis factor-α(TNF-α) in two groups increased at 24h after operation, and were lower in group Ⅱ than those in group Ⅰ, and the differences were statistically significant(P<0.05). There was no significant difference in the total incidence of adverse reactions between the groups(P>0.05). Conclusion Compared with 0.1mg/kg of oxycodone hydrochloride, 0.15mg/kg of oxycodone hydrochloride is more beneficial to relieve postoperative pain,reduce postoperative stress and inflammatory response in patients undergoing laparoscopic hysterectomy, without increasing the risk of adverse drug reactions.
