摘要
目的 分析甲状腺功能对代谢综合征患者糖脂代谢的影响。方法 选取2020年1月至2022年1月浙江大学医学院附属第二医院临平院区收治的389例代谢综合征患者为研究对象,依据甲状腺功能分为甲减组(n=54)、亚临床甲减组(n=62)、甲状腺功能正常组(n=166)、亚临床甲亢组(n=58)、甲亢组(n=49)。根据患者HbA1c水平,分为≤7%组(n=25)、>7%且≤10%组(n=51)、>10%且≤13%组(n=144)、>13%组(n=169);根据HDL-C水平,分为<0.89mmol/L组(n=95),≥0.89且<1.62mmol/L组(n=191)、≥1.62mmol/L组(n=103),根据LDL-C水平,分为<2.00mmol/L组(n=86)、≥2.00且<3.10mmol/L组(n=179)、≥3.10mmol/L组(n=124)、根据TC水平,分为<2.25mmol/L组(n=104)、≥2.25且<5.71mmol/L组(n=189)、≥5.71mmol/L组(n=96),根据TG水平,分为<0.60mmol/L组(n=98)、≥0.60且<1.70mmol/L组(n=176)、≥1.70mmol/L组(n=115)。比较不同甲状腺功能代谢综合征患者的空腹血糖(fasting plasma glucos,FPG)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、高密度脂蛋白胆固醇(high density liptein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、总胆固醇(serum total cholesterol,TC)、三酰甘油(triglyceride,TG)水平。并比较不同HbA1c水平及不同血脂水平代谢综合征患者的甲状腺激素水平,分析甲状腺激素水平与血糖、血脂指标的相关性。结果 各组HDL-C、LDL-C、TC、TG水平比较,甲减组最高,其次为亚临床甲减组,再次为甲状腺功能正常组,然后为亚临床甲亢组,甲亢组最低,差异有统计学意义(P<0.05)。HbA1c水平≤7%组的游离三碘甲状原氨酸(free triiodothyronine,FT3)、总三碘甲状腺原氨酸(total-triiodothyronine,TT3)、促甲状腺激素(thyroid-stimulatinghormone,TSH)水平均高于其他组,差异有统计学意义(P<0.05)。HDL-C<0.89mmol/L组、LDL-C<2.00mmol/L组、TC<2.25mmol/L组、TG<0.60mmol/L组的游离甲状腺素(free thyroxine,FT4)、TT3水平均低于同指标下其他组,差异�
Objective To analyze the effect of thyroid function on glucose and lipid metabolism in patients with metabolic syndrome. Methods A total of 389 patients with metabolic syndrome admitted to the Second Affiliated Hospital of Zhejiang University Medical College Linping Campus from January 2020 to January 2022 were selected as the study subjects. According to thyroid function, the patients were divided into hypothyroidism group(n=54), subclinical hypothyroidism group(n=62),normal thyroid function group(n=166), subclinical hyperthyroidism group(n=58), hyperthyroidism group(n=49). According to glycosylated hemoglobin(HbA1c) levels, the patients were divided into ≤ 7% group(n=25), >7% and ≤ 10% group(n=51), >10% and ≤13% group(n=144), and >13% group(n=169);according to high density liptein cholesterol(HDL-C)levels, the patients were divided into <0.89mmol/L group(n=95), ≥ 0.89mmol/L and <1.62mmol/L group(n=191),≥1.62mmol/L group(n=103);according to low-density lipoprotein cholesterol(LDL-C) levels, the patients were divided into <2.00mmol/L group(n=86), ≥2.00mmol/L and <3.10mmol/L group(n=179), ≥3.10mmol/L group(n=124), according to serum total cholesterol(TC) level, the patients were divided into <2.25mmol/L group(n=104), ≥2.25mmol/L and <5.71mmol/L group(n=189), ≥5.71mmol/L group(n=96);according to triglyceride(TG) level, the patients were divided into <0.60mmol/L group(n=98), ≥0.60mmol/L and <1.70mmol/L group(n=176), ≥1.70mmol/L group(n=115). To compare the levels of fasting plasma glucose(FPG), HbA1c, HDL-C, LDL-C, TC and TG in patients with different thyroid functional metabolic syndrome.We also compared the thyroid hormone levels of patients with metabolic syndrome with different levels of glycated hemoglobin(HbA1c) and blood lipids, and analyzed the correlation between thyroid hormone levels and blood glucose, blood lipid indicators.
