摘要
目的:观察不同剂量盐酸罗哌卡因腰硬联合麻醉在剖宫产手术中的麻醉效果及对血流动力学的影响。方法:选取2019年1月-2021年12月江阴北国医院收治的行剖宫产手术的孕产妇50例为观察对象,根据随机数字表法分为研究组与对照组,各25例。对照组实施大剂量盐酸罗哌卡因腰硬联合麻醉,研究组实施小剂量盐酸罗哌卡因腰硬联合麻醉。比较两组患者麻醉效果、血流动力学指标。结果:两组患者麻醉到手术开始时间、感觉阻滞起效时间、运动阻滞起效时间、手术时间、Bromage评分与麻醉阻滞效果评分比较,差异无统计学意义(P>0.05);胎儿娩出后,研究组患者HR、MAP与SpO_(2)水平低于对照组,差异有统计学意义(P<0.05)。结论:不同剂量盐酸罗哌卡因腰硬联合麻醉在剖宫产手术中均有良好的麻醉效果表现,小剂量盐酸罗哌卡因稳定患者的血流动力学水平的效果表现更好。
Objective:To observe the anesthetic effect of different doses of ropivacaine hydrochloride lumbar hard combined anesthesia in cesarean section and the effect on hemodynamics.Methods:Fifty cases of pregnant women admitted to Jiangyin Beiguo Hospital who performed cesarean section from January 2019 to December 2021 were selected as observation subjects,and 25 cases each were divided into study group and control group according to the random number table method.The control group was administered high-dose ropivacaine hydrochloride lumbar-hard combined anesthesia,and the study group was administered low-dose ropivacaine hydrochloride lumbar-hard combined anesthesia.The anesthetic effect and hemodynamic indexes of patients in the two groups were compared.Results:There was no statistically significant difference in the time from anesthesia to the start of surgery,time to onset of sensory block,time to onset of motor block,time to surgery,Bromage score and anesthesia block effect score between the two groups(P>0.05).After delivery of the fetus,the HR,MAP and SpO_(2) levels of patients in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Different doses of ropivacaine hydrochloride in combined lumbar and rigid anesthesia showed good anesthetic performance in cesarean section,and the smaller dose of ropivacaine hydrochloride had better performance in stabilizing the hemodynamic level of patients.
