摘要
目的:探究经方真武汤配合穴位贴敷治疗脾肾阳虚型膝关节骨性关节炎(KOA)的临床效果。方法:选择2021年9月-2022年3月于九江市中医医院接受治疗的脾肾阳虚型KOA患者60例,采用随机数字表法分为治疗组(n=30)及对照组(n=30)。对照组采用穴位贴敷治疗,治疗组在对照组基础上配合经方真武汤治疗,两组均治疗4周。对比两组临床疗效、中医症候积分、血液流变学指标、炎症因子水平及生活质量。结果:相比对照组,治疗组总有效率更高(P<0.05);治疗后,相比对照组,治疗组各中医症候积分、全血低切黏度、全血高切黏度、血浆黏度、白细胞介素-17(IL-17)、白细胞介素-1β(IL-1β)水平均更低,关节炎影响评估表2(AIMS2)各维度评分及总分均更高(P<0.05)。结论:经方真武汤配合穴位贴敷在脾肾阳虚型KOA患者治疗中可获得较好的临床效果,有助于缓解患者膝关节不适症状,减轻炎症反应,改善血液流变学指标,提高生活质量,值得推广应用。
Objective:To explore the clinical effect of Jingfang Zhenwu Decoction combined with acupoint application in the treatment of knee osteoarthritis(KOA)with spleen and kidney yang deficiency.Method:A total of 60 KOA patients with spleen and kidney yang deficiency who were treated in Jiujiang Hospital of Traditional Chinese Medicine from September 2021 to March 2022 were randomly divided into treatment group(n=30)and control group(n=30).The control group was treated with acupoint application,and the treatment group was treated with Jingfang Zhenwu Decoction on the basis of the control group.Both groups were treated for 4 weeks.The clinical efficacy,TCM syndrome scores,blood rheology indexes,inflammation factor levels and quality of life were compared between the two groups.Result:Compared with the control group,the total effective rate in the treatment group was higher(P<0.05).After treatment,compared with the control group,the scores of TCM syndromes,low-shear viscosity of whole blood,high-shear viscosity of whole blood,plasma viscosity,interleukin-17(IL-17)and interleukin-1β(IL-1β)levels in the treatment group were lower,and the each dimension scores and total scores of arthritis impact measurement scales 2(AIMS2)were higher(P<0.05).Conclusion:Jingfang Zhenwu Decoction combined with acupoint application in the treatment of KOA patients with spleen and kidney yang deficiency achieved good clinical effect,which helped to alleviate the symptoms of knee joint discomfort,reduce inflammatory reaction,improve blood rheology index,improve the quality of life,and it is worthy of promotion and application.
