摘要
目的探讨达格列净在老年糖尿病合并慢性心力衰竭患者中的应用。方法选取诊断为老年糖尿病合并射血分数减低的心力衰竭(HFrEF)患者共60例,根据病情予以降糖及抗心衰常规治疗。其中对照组予以常规治疗,观察组在常规治疗的基础上予以达格列净进行治疗,比较2组治疗前后N末端B型钠尿肽前体(NT-proBNP)、6 min步行距离(6MWD)、左室射血分数(LVEF)等指标结果变化情况。结果观察组治疗后NT-proBNP水平明显降低,与对照组比较差异有统计学意义(P<0.05);观察组治疗后6MWD较治疗前升高,与对照组比较,差异有统计学意义(P<0.05);2组LVEF治疗前后变化差异无统计学意义(P>0.05)。结论达格列净能有效提高老年糖尿病合并慢性心力衰竭患者的心功能,并且药物不良反应发生较低。
Objective To explore the application of Dapagliflozin in elderly patients with diabetes complicated with chronic heart failure.Methods A total of 60 patients with heart failure(HFrEF)who were diagnosed as elderly diabetes with reduced ejection fraction(EF)and treated in our hospital were included in the study.According to their conditions,the patients were received conventional treatment of hypoglycemia and anti-heart failure,and they were divided into two groups according to the principle of randomization.The control group received conventional treatment and the observation group was treated with Dapagliflozin on the basis of conventional treatment,and the changes of n-terminal B-type natriuretic peptide precursor(NT-probNP),6-min walking distance(6MWD),LVEF and other indicators were compared before and after treatment in the two groups.Results NT-proBNP in the observation group was significantly lower than that in the control group(P<0.05).The 6MWD of the observation group after treatment was higher than that before treatment,and the difference was statistically significant compared with that of the control group(P<0.05).There was no statistically significant change in LVEF before and after treatment in both groups(P>0.05).Conclusion Dapagliflozin has obvious clinical efficacy in the treatment of senile diabetic patients with chronic heart failure,and it can effectively improve the heart function of senile patients with chronic heart failure,and the incidence of adverse drug reactions is low.
