摘要
目的探讨使用青少年型髓内钉治疗大龄儿童股骨干骨折近期疗效。方法选取2019年11月至2021年1月,我院创伤骨科收治的14例股骨干骨折患儿,使用青少年型髓内钉治疗固定股骨干骨折,术后均进行对症治疗并指导患儿进行相应的下肢功能康复训练。随访评价患肢髋关节、膝关节功能评分,测量双侧肢体长度,颈干角、股骨颈粗细有无变化等。结果术后随访6~20个月,平均14.3个月,所有患儿骨折均愈合,时间11~28周,平均16.3周。无一例出现切口问题或骨折端不愈合、内固定失败、股骨头坏死等并发症,术后6个月复查患侧髋、膝关节活动度良好,与健侧肢体相比未发生旋转畸形,至末次随访时患肢长度、颈干角、股骨颈直径与健侧相比差异无统计学意义(P>0.05)。结论采用青少年型髓内钉内固定治疗大龄儿童股骨干骨折是一种安全而有效的选择,符合创伤快速康复原则理念,应作为治疗大龄儿童股骨干骨折治疗方法之一。
Objective To research the short-term effectiveness of adolescent intramedullary nail for femoral shaft fractures in elder children.Methods From November 2019 to January 2021,14 patients with femoral shaft fracture underwent treatment with adolescent intramedullary nail.Functional exercises of lower limbs were instructed.The hip joint and knee function scores of the affected limb were evaluated.Length of bilateral limbs,collo-diaphyseal angle and femoral neck thickness were measured.Results All patients were followed up for 6-20 months,with an average of 14.3 months.All fractures healed from 11 weeks to 28 weeks,with an average of 16.3 weeks.No infection,nonunion,implant failure or osteonecrosis was observed.The hip and knee ROM recovered without obvious rotation deformity 6 months after the operation.The differences in length of bilateral limbs,collo-diaphyseal angle and femoral neck thickness between the affected side and healthy side were not statistically significant(P>0.05).Conclusions Adolescent intramedullary nail is safe and effective in the treatment of femoral shaft fracture in elder children,which conforms to ERAS principle.It should be recommended in clinical practice.
