肺炎克雷伯菌感染的临床分布特征及耐药性分析

摘要 目的分析肺炎克雷伯菌感染的临床分布特征以及耐药性。方法抽取2018年9月至2020年6月南阳市第一人民医院收治的35例肺炎克雷伯菌感染患者作为研究对象,对其痰液及血液标本进行细菌培养并记录抗菌药物药敏结果,记录患者的标本来源及科室来源,分析肺炎克雷伯菌对抗菌药物的耐药性。结果肺炎克雷伯菌的标本来源中以痰液标本为主,占54.29%(19/35),其次为尿液标本(17.14%,6/35)和咽拭子标本(11.43%,4/35);肺炎克雷伯菌的科室来源以呼吸内科为主,占60.00%(21/35),其次为重症监护室(22.86%,8/35)。肺炎克雷伯菌在临床上的耐药状况较为复杂,对头孢类药物的耐药性最高,均为20.00%(7/35);对于亚胺培南、美罗培南、阿米卡星的耐药性较低,均为2.86%(1/35)。结论医务人员要根据肺炎克雷伯菌感染的临床分布特征做好有效的预防,设置肺炎克雷伯菌感染隔离区,并强化临床日常管理,及时根据病情调整抗菌药物的应用,有效预防肺炎克雷伯菌的耐药性。 Objective To analyze the clinical distribution characteristics and drug resistance of Klebsiella pneumoniae infection.Methods Thirty-five patients with Klebsiella pneumoniae infection admitted to Nanyang First People’s Hospital from September 2018 to June 2020 were selected as the study subjects.The sputum and blood samples from these patients were cultured,and the antimicrobial susceptibility results were recorded;the source of samples and departments distribution were recorded,and the resistance of Klebsiella pneumoniae to antibiotics was analyzed.Results Sputum samples were the main source of Klebsiella pneumoniae,accounting for 54.29%(19/35),followed by urine samples(17.14%,6/35)and throat swab samples(11.43%,4/35).The department source of Klebsiella pneumoniae was mainly respiratory department,accounting for 60.00%(21/35),followed by intensive care unit(22.86%,8/35).The clinical drug resistance of Klebsiella pneumoniae was complicated,and the drug resistance rates to cephalosporins were the highest,all were 20.00%(7/35);and the resistance rate to imipenem,meropenem and amikacin were low,all were 2.86%(1/35).Conclusions Medical staff should effectively perform prevention according to the clinical distribution characteristics of Klebsiella pneumoniae infection,set up the isolation area of Klebsiella pneumoniae infection,and strengthen the daily clinical management.The application of antibiotics should timely be regulated according to the condition,so as to effectively prevent the drug resistance of Klebsiella pneumoniae.
