摘要
目的通过对胰腺实性占位进行超声造影,探讨其定量参数在胰腺良恶性病灶鉴别诊断中的应用价值。方法回顾性分析2018年1月至2021年9月就诊于首都医科大学附属北京友谊医院的37例胰腺实性占位患者,经超声造影检查并绘制时间强度曲线进行定量分析,比较上升时间、上升支斜率及相对增强强度在胰腺实性占位中的差异。结果在时间强度曲线定量分析中,恶性病变的曲线上升时间较良性病变延长[(11.16±6.91)s vs.(6.40±3.15)s],上升支斜率较良性病变者小[(11.27±6.34)Lev/s vs.(15.84±10.65)Lev/s],差异均有统计学意义(P<0.05)。受试者工作特征曲线分析中,当胰腺实性占位时间强度曲线的上升时间曲线下面积为0.782(P=0.005),当界值为7.91 s时,其敏感度、特异度、阳性预测值、阴性预测值分别为70.83%、84.62%、89.47%、61.11%;而上升支斜率曲线下面积为0.615(P=0.252),则该指标对判断胰腺实性肿物良恶性所起到作用有限。结论超声造影定量参数可为胰腺实性占位的良恶性鉴别提供参考依据。
Objective To explore the value of quantitative parameters in the differential diagnosis of benign and malignant pancreatic solid lesions by contrast-enhanced ultrasonography(CEUS).Methods From January 2018 to September 2021,37 patients with solid pancreatic lesions in Beijing Friendship Hospital Affiliated to Capital Medical University were retrospectively analyzed.CEUS examination and time intensity curve(TIC)were drawn for quantitative analysis,and the differences of rise time,raise rate and relative rise intensity in benign and malignant pancreatic solid lesions were compared.Results The rise time of malignant lesions was longer than that of benign lesions[(11.16±6.91)s vs.(6.40±3.15)s],and the raise rate was smaller than that of benign lesions[(11.27±6.34)Lev/s vs.(15.84±10.65)Lev/s]in the quantitative analysis of TIC,the differences were statistically significant(P<0.05).In the analysis of receiver operating characteristic curve(ROC)of subjects,the area under the curve(AUC)was 0.782(P=0.005)in rise time,and when the rise time boundary of pancreatic solid leision in TIC was 7.91s,the sensitivity,specificity,positive predictive value and negative predictive value were 70.83%,84.62%89.47%and 61.11%respectively.The AUC of raise rate was 0.615(P=0.252),so this parameter played a limited role in judging the benign and malignant solid tumors of the pancreas.Conclusion The quantitative parameters of CEUS can provide a reference basis for the differentiation of benign and malignant pancreatic solid lesions.
