摘要
目的:探讨分析针灸联合康复理疗对老年脑出血患者认知功能、脑血管功能的临床实际治疗效果。方法:选取鄂州二医院于2019年9月―2020年12月收治的126例老年脑出血患者作为研究对象,并通过简单随机化分组的方法将其分为两组,其中对照组63例,观察组63例。对照组患者采取常规对症治疗,观察组患者在常规对症治疗的基础上加以针灸联合康复理疗治疗,评估并对比两组患者在不同治疗方式治疗前后的认知功能评分、脑血管功能及中医证候积分。结果:对比两组患者治疗前后的认知功能,发现观察组患者治疗后的简易精神状态评价量表(Mini-mental State Examination,MMSE)评分和美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale,NIHSS)评分较对照组患者均显著降低,差异具有统计学意义(P<0.05);对比两组患者治疗前后的脑血管功能,发现治疗后观察组患者的脑血管流速和脑血管流量均明显高于对照组,血管外周阻力和动态阻力明显低于对照组,差异具有统计学意义(P<0.05);对比两组的中医证候积分,发现治疗后观察组患者的半身不遂等各类主症及舌强语謇等次症积分均显著低于对照组,差异具有统计学意义(P<0.05)。结论:针灸联合康复理疗治疗能显著提高老年脑出血患者的认知功能,有效改善患者的脑血管功能并优化中医证候积分。
Objective:To analyze the clinical practical therapeutic effects of acupuncture plus rehabilitation physiotherapy on cognitive function and cerebrovascular function of elderly patients with intracerebral hemorrhage.Methods:A total of 126 elderly patients with intracerebral hemorrhage(ICH) admitted to Ezhou Second Hospital from September 2019 to December 2020 were selected as the study subjects,and they were divided into two groups by the method of simple randomization,including the control group with 63 cases and the observation group with 63 cases.Patients in the control group were treated by conventional symptomatic treatment,patients in the observation group were treated by acupuncture plus rehabilitation physiotherapy on the basis of conventional symptomatic treatment;and the cognitive function scores,cerebrovascular function,and TCM syndrome scores in the two groups before and after treatment were evaluated and compared.Results:Comparing the post-treatment cognitive function between the two groups,we found that the mini mental state examination(MMSE) scores and National Institutes of Health Stroke Scale(NIHSS) scores in the observation group were significantly lower than those the control group,with significant differences(P<0.05).The post-treatment cerebrovascular velocity and flow of the patients in the observation group were significantly higher than the control group,and the peripheral resistance and dynamic resistance of the vessels were significantly lower than the control group,with statistically significant differences(P<0.05).Comparing the TCM syndrome scores in the two groups,we found that after treatment,the scores of all kinds of primary syndromes,such as hemiplegia,and secondary syndromes,such asstiff tongue,were significantly lower in the observation group than in the control group,with significant differences(P<0.05).Conclusion:Acupuncture plus rehabilitation physiotherapy can significantly improve the cognitive function of elderly patients with intracerebral hemorrhage,effectively promote the
