血糖升高与消化系统肿瘤标志物的相关性以及趋势分析

摘要 目的探究糖尿病患者血糖升高与消化系统肿瘤标志物的相关性以及趋势分析。方法收集67例糖尿病患者(diabetes,DM)作为实验组,并收集144例健康体检者作为对照组,根据HbAlc水平,将其分为正常人组(healthy control,HC)70例,糖尿病前期组(prediabetic)74例。分别检测血清中癌胚抗原(CEA)、糖类抗原CA19-9、糖类抗原CA50以及糖类抗原CA72-4的水平,并且回顾性收集患者的临床信息,包括空腹血糖、血清总胆固醇、肌酐等。结果糖尿病患者血清中CEA、CA19-9、CA50的表达水平与正常对照组相比差异有统计学意义(P<0.05),相关性分析显示,CEA(r=0.147)、CA19-9(r=0.261)、CA50(r=0.226)水平与空腹血糖水平呈正相关。ROC曲线分析表明,血清中的CEA、CA19-9、CA50对于糖尿病具有一定的监测价值,AUC分别为0.651、0.666和0.644。结论糖尿病血糖升高可能增加了消化系统肿瘤发生的可能性,提示DM患者应关注肿瘤标志物的检测与随访。 Objective To explore the correlation and trend of elevated blood sugar and digestive system tumor markers in diabetic patients.Methods A total of 67 diabetic patients(Diabetes,DM)were collected as the experimental group,while 144 healthy subjects were collected as the control group.According to the level of HbAlc,these patients were divided into 70 normal people(Healthy control,HC)and 74 prediabetic patients.Serum levels of carcinoembryonic antigen(CEA),carbohydrate antigen CA19-9,carbohydrate antigen CA50 and carbohydrate antigen CA72-4 were detected respectively,and the clinical information of patients,including fasting blood glucose,serum total cholesterol,and creatinine,were also collected retrospectively.Results Expression levels of CEA,CA19-9 and CA50 in serum of diabetic patients were significantly different from those of the normal control group(P<0.05).Correlation analysis showed that CEA(r=0.147),CA19-9(r=0.261),and CA50(r=0.226)levels were positively correlated with fasting blood glucose levels.ROC curve analysis suggested that CEA,CA19-9 and CA50 in serum had certain monitoring value for diabetes,with AUCs of 0.651,0.666 and 0.644,respectively.Conclusion Elevated blood sugar in diabetes may increase the risk of digestive system tumors,suggesting that it is important for DM patients to pay attention to the detection and follow-up of tumor markers.
