超声引导下动脉穿刺置管术安全性和效果的Meta分析 被引量:2

摘要 目的通过Meta分析探讨经由超声引导行梯动脉穿刺置管术的安全性和效果。方法使用计算机检索中国知网(CNKI)、万方、PubMed、Web of Science,Embase等中、英文数据库,搜集2022年3月之前公开发表的超声引导下行棧动脉穿刺置管的相关文献。应用Cochrane工具评估文献偏倚风险,采用RevMan 5.4.1软件进行Meta分析。结果共纳入26篇文献,共有3107例患者需要行桡动脉穿刺置管,其中超声引导组(试验组)共1558例,触摸法(对照组)共1549例。Meta分析结果显示:首次动脉穿刺置管成功率试验组是对照组的1.46倍[RR=1.46,95%CI(1.36,1.56),P<0.00001];穿刺置管次数试验组比对照组少用0.56次[MD=-0.56,95%CI(-0.74,-0.38),P<0.00001];穿刺成功时间试验组比对照组少用39.45 s[MD=-39.45,95%CI(-51.82,-27.09),P<0.00001];并发症发生率试验组是对照组的0.27倍[RR=0.27,95%CI(0.21,0.34),P<0.00001]。结论超声引导动脉穿刺置管在首次成功率、穿刺次数、穿刺时间及并发症发生率方面均优于触摸法。 Objective To systematically evaluate the safety and effectiveness of arterial puncture placement guided by ultrasound.Methods Chinese and English databases such as China Knowledge Network(CNKI),Wanfang,PubMed,Web of Science,and Embase were searched using computers to collect relevant literature on published ultrasound-guided invasive arterial puncture cannulation before March 2022.Cochrane tools were applied to assess the risk of literature bias,and Meta-analysis was perfbnned using RevMan 5.4.1 software.Results A total of 26 publications were included,with a total of 3107 patients requiring operational arterial puncture placement,including 1558 cases in the ultrasound-guided group(trial group)and 1549 cases in the touch method(control group).Meta-analysis showed that the success rate of first arterial puncture placement was 1.46 times higher in the trial group than that in the control group[RR=1.46,95%CI(1.36,1.56),P<0.00001].The number of puncture placements was 0.56 less in the trial group than that in the control group[MD=-0.56,95%CI(-0.74,-0.38),P<0.00001].The time to successful puncture was 39.45 seconds less in the trial group than that in the control group[MD=-39.45,95%CI(-51.82,-27.09),P<0.00001].The complication rate was 0.27 times higher in the test group than that in the control group[RR=0.27,95%CI(0.21,0.34),P<0.00001].Conclusion Ultrasound-guided arterial puncture placement is superior to the touch method in terms of first-time success rate,number of punctures,puncture time and complication rate.
