摘要
目的分析高频超声鉴别诊断小儿急性阑尾炎和急性肠系膜淋巴结炎的临床价值。方法120例疑似小儿急性阑尾炎、急性肠系膜淋巴结炎患儿,均接受高频超声检查。以病理组织学检查结果为金标准,分析高频超声鉴别诊断两种疾病的阳性预测值、灵敏度及特异度。结果经过病理组织学检查,120例患儿中54例确诊为急性阑尾炎,66例确诊为急性肠系膜淋巴结炎。120例患儿经高频超声诊断,其中急性阑尾炎阳性50例,阴性70例;急性肠系膜淋巴结炎阳性62例,阴性58例。高频超声对小儿急性阑尾炎的阳性预测值、灵敏度与特异度分别为96.00%(48/50)、88.89%(48/54)、96.97%(64/66),对急性肠系膜淋巴结炎的阳性预测值、灵敏度与特异度分别为93.55%(58/62)、87.88%(58/66)、92.59%(50/54)。结论高频超声对小儿急性阑尾炎与急性肠系膜淋巴结炎具有较高的鉴别诊断价值,可以作为临床诊断儿童急腹症的重要辅助措施加以推广。
Objective To analyze the clinical value of high-frequency ultrasound in differential diagnosis of acute appendicitis and acute mesenteric lymphadenitis in children.Methods All 120 children with suspected acute appendicitis and acute mesenteric lymphadenitis underwent high-frequency ultrasound examination.The positive predictive value,sensitivity and specificity of high-frequency ultrasound for the differential diagnosis of the two diseases were analyzed using the pathological histological findings as the gold standard.Results After pathological histological examination,54 of the 120 children were diagnosed with acute appendicitis and 66 cases with acute mesenteric lymphadenitis.Among the 120 children diagnosed by high-frequency ultrasound,50 cases were positive and 70 cases were negative for acute appendicitis;62 cases were positive and 58 cases were negative for acute mesenteric lymphadenitis.The positive predictive value,sensitivity and specificity of high-frequency ultrasound for acute appendicitis were 96.00%(48/50),88.89%(48/54)and 96.97%(64/66),and the positive predictive value,sensitivity and specificity for acute mesenteric lymphadenitis were 93.55%(58/62),87.88%(58/66)and 92.59%(50/54).Conclusion High-frequency ultrasound has high reference value in the differential diagnosis of acute appendicitis and acute mesenteric lymphadenitis in children,which can be used as an important auxiliary measure for clinical diagnosis.
