摘要
目的观察通窍活血汤联合中医康复治疗对脑卒中患者神经功能恢复的影响。方法将我院就诊的96例脑卒中患者按单双号法随机分为观察组与对照组各48例,对照组在常规治疗基础上联合中医康复治疗,观察组在对照组基础上加用通窍活血汤,比较两组患者临床疗效,治疗前后的中医症候评分、神经功能缺损程度、认知功能、血液流变学指标。结果治疗后,观察组总有效率显著高于对照组(P<0.05);两组中医症候评分较治疗前显著提高(P<0.05),且观察组显著高于对照组(P<0.05);两组神经缺损程度(NIHSS)评分较治疗前显著降低(P<0.05),认知评估(MoCA)评分较治疗前显著提高(P<0.05),且两组差异显著(P<0.05);两组纤维蛋白原、甘油三酯、血浆黏度较治疗前显著降低(P<0.05),且观察组显著低于对照组(P<0.05)。结论通窍活血汤联合中医康复治疗脑卒中疗效显著,能改善患者神经功能,提高患者认知功能,改善患者血液流变学,值得临床推广。
Objective To observe the effects of Tongqiao Huoxue Decoction combined with TCM rehabilitation on the recovery of neurological function in patients with stroke.Methods Ninety-six patients with stroke treated in the hospital were randomly assigned to observation group and control group according to the odd-even number method,48 cases each.Control group was combined with TCM rehabilitation therapy on the basis of routine treatment,and observation group was additionally treated with Tongqiao Huoxue Decoction on the basis of control group.The clinical efficacy,as well as scores of TCM symptoms,neurological impairment degree,cognitive function and hemorheological indicators before and after treatment were compared between the two groups.Results After treatment,the total effective rate in observation group was significantly higher than that in control group(P<0.05);TCM syndrome scores were significantly enhanced compared to before treatment in both groups(P<0.05),and were significantly higher in observation group than in control group(P<0.05);neurological impairment degree(National Institutes of Health Stroke Scale,NIHSS)scores was significantly reduced compared with that before treatment(P<0.05)while the Montreal cognitive assessment(MoCA)scores was significantly risen compared with that before treatment in both groups(P<0.05),and the differences between the two groups were significant(P<0.05);the fibrinogen,triglyceride and plasma viscosity were significantly lower after treatment than those before treatment in both groups(P<0.05),and the above three indicators of observation group were significantly lower compared to control group(P<0.05).Conclusion Tongqiao Huoxue Decoction combined with TCM rehabilitation has a significant efficacy in treatment of stroke.It can improve the neurological function,enhance the cognitive function and promote the hemorheology of patients,thus it is worthy of clinical promotion.
