虚拟现实技术对脑卒中患者功能平衡及下肢运动功能影响的Meta分析 被引量:1

Effects of Virtual Reality Technology on The Balance and Lower Limb Extrimity of Patients with Post-stroke Dysfunction:A Meta-Analysis of Randomized Controlled Trial

摘要 目的:评价虚拟现实技术对卒中后功能障碍患者平衡及下肢运动功能的疗效。方法:在中国知网、万方、维普、PubMed、Web of science、Medline数据库检索关于虚拟现实技术改善卒中患者平衡及下肢功能的随机对照试验(RCT),年限设置为建库至2021年8月。根据纳排标准逐步进行筛选,提取数据,采用Revman 5.4软件对符合标准的RCT平衡功能、下肢功能、TUGT等指标数据进行Meta分析。结果:共纳入28项RCT研究,样本量共1344例,试验组664例,对照组680例。森林图结果显示:虚拟现实康复训练组的Berg平衡量表得分(SMD=1.43,95%CI [1.07,1.80],Z=7.68,P<0.05),Fugl-Meyer量表下肢评分(SMD=1.37,95%CI [0.91,1.82],Z=5.88,P<0.05),起立-行走试验(SMD=-1.07,95%CI [-1.58,-0.57],Z=4.14,P<0.05)等指标均优于对照组。亚组分析显示,在下肢运动功能和短时步行速度部分,最大效应量与更长的干预周期相对应;但在平衡得分部分,干预4周的效应分别大于12周、6~8周。结论:虚拟现实康复技术能够改善卒中患者平衡及下肢运动功能,且在一定程度上优于传统康复训练,但在干预强度与时限上如何达到效益最大化尚未定论。 Objective:To evaluate the effect of virtual reality technology on the balance and lower limb motor function of patients with post-stroke dysfunction.Methods:The randomized controlled trials(RCTs) on the improvement of balance and lower limb function of stroke patients with virtual reality technology of CNKI,Wanfang,Weipu,PubMed,Web of science,and Medline databases were electronically searched,with limition of database from the establishment to August 2021.The screening and data was carried out and extracted step by step according to the acceptance and exclusion criteria.Revman 5.4 software was used to conduct meta-analysis.Results:A total of 28 RCTs involving 1344 patients were included,with664 cases in the experimental group and 680 cases in the control group.The results of the forest plot showed that the score of BBS(SMD=1.43,95%CI [1.07,1.80],Z=7.68,P<0.05),the score of FMA-LE(SMD=1.37,95%CI [0.91,1.82],Z=5.88,P<0.05),the score of TUGT(SMD=-1.07,95%CI [-1.58,-0.57],Z=4.14,P<0.05)of virtual reality rehabilitation training group were superior to the control group.Subgroup analysis showed that in the parts of lower limb motor function and short-term walking speed,the maximum effect size corresponds to a longer intervention period;but in the balance score part,the effects of intervention for 4 weeks were superior than 12 weeks and 6-8 weeks,respectively.Conclusion:Virtual reality rehabilitation technology can improve the balance and lower limb motor function of stroke patients,and is better than traditional rehabilitation training to a certain extent.However,how to maximize the benefits in terms of intervention intensity and period has not yet been determined.
