应用斑点追踪成像技术对肥厚梗阻型心肌病患者左心室功能的评价研究 被引量:2

摘要 目的:探究斑点追踪成像(STI)技术对肥厚梗阻型心肌病(HOCM)患者左心室功能的评估价值。方法:选取医院诊治的52例HOCM患者,将其纳入HOCM组,同期选取36名健康体检者,将其纳入健康对照组。两组均采用STI技术,比较超声心动图指标,并以STI获得左心室各节段纵向、径向、周向收缩期峰值应变及应变率,以受试者工作特征(ROC)曲线评估STI对HOCM左心室功能的诊断价值。结果:HOCM组与健康对照组的左心室舒张末期前后径(LVEDD)、左心室后壁舒张末期厚度(LVPWT)及左心室射血分数(LVEF)差异均无统计学意义;但HOCM组的室间隔舒张末期厚度(IVST)和频谱多普勒估测静息左心室流出道压力阶差(LVOT-PG)均显著高于健康对照组(t=10.591,t=7.075;P<0.05)。HOCM组纵向应变(LS)和周向应变(CS)均高于健康对照组(t=6.300,t=6.082;P<0.05),纵向应变率(LSr)、径向应变(RS)、径向应变率(RSr)及周向应变率(CSr)值均低于健康对照组(t=6.900,t=4.136,t=5.312,t=7.390;P<0.05);HOCM组基底段的前壁、侧壁及后壁的LSr均大于健康对照组(t=3.830,t=2.680,t=2.162;P<0.05),基底段及中间段的前壁、侧壁及后壁的RSr小于健康对照组(t基底段=2.987,t=3.105,t=2.783;t中间段=2.635,t=2.594,t=2.097;P<0.05),基底段的侧壁、后壁的CSr及中间段前壁、侧壁的CSr均大于健康对照组(t=2.370,t=2.128,t=2.189,t=2.516;P<0.05)。ROC曲线分析显示,LS、LSr、RS、RSr、CS和CSr评估HOCM的ROC曲线下面积(AUC)分别为0.826、0.808、0.730、0.751、0.806和0.824。结论:STI可有效评估HOCM患者左心室心肌节段应变及应变率变化情况,对于患者左心室功能的评估具有较高的价值。 Objective: To explore the assessment value of speckle tracking imaging(STI) on left ventricular function in patients with hypertrophic obstructive cardiomyopathy(HOCM). Methods: A total of 52 HOCM patients who were diagnosed and treated in hospital were selected, and they were divided into HOCM group, and 36 persons who underwent heath physical examination were divided into health control group at the same time. Both two groups adopted STI, and the indicators of ultrasound cardiogram were compared. The peak strains and strain rates of each segment of left ventricular sinister at the systoles on longitudinal direction, radial direction and circumference direction were obtained by using STI. The receiver operating characteristics(ROC) curve was used to assess the diagnostic value of STI on the left ventricular function of HOCM patients. Results: There were no significant differences in left ventricular end-diastolic dimension(LVEDD), left ventricular posterior wall thickness(LVPWT) and left ventricular ejection fraction(LVEF) between HOCM group and control group(P>0.05). However, the interventricular septum thickness(IVST) at end diastole and left ventricular outflow tract pressure gradient(LVOT-PG) of HOCM group were significantly higher than those of control group(t=10.591, t=7.075, P<0.05), respectively. The longitudinal strain(LS) and circumference strain(CS) in HOCM group were significantly higher than those in health control group(t=6.300, t=6.082, P<0.05), and LS rate(LSr), radial strain(RS), RS rate(RSr) and CS rate(CSr) of HOCM group were significantly lower than those in health control group(t=6.900, t=4.136, t=5.312, t=7.390, P<0.05),respectively. The LSr of the front wall, side wall and back wall of the basal section in HOCM group were significantly higher than those in health control group(t=3.830, t=2.680, t=2.162, P<0.05), and the RSr of the front wall, side wall and back wall of the basal section and the middle section of former were significantly lower than those of health control group(tbasal sect
