磁共振SWI在诊断脑血管病微出血灶中的应用价值

摘要 目的探讨磁共振SWI在诊断脑血管病微出血灶中的应用价值。方法选取徐州市第六人民医院在2019年1月—2021年12月收治的68例脑出血病合并微出血患者,均进行MRI常规序列及DWI、SWI序列扫描,对比各序列的脑微出血灶检出情况、脑微出血灶的各序列影像学表现、各序列脑内不同部位及微出血灶检出数目情况。结果在脑微出血灶的阳性检出率中,SWI阳性检出率为100.0%,其诊断结果相较于MRI常规序列的80.9%和DWI序列的94.1%均更高,差异有统计学意义(χ^(2)=4.121、14.374,P=0.042、0.001),DWI序列与MRI常规序列比较,DWI序列检出率更高,差异有统计学意义(P<0.05)。SWI序列下表现出直径<10 mm的小圆形或卵形的低信号缺失,而多层面上无连续性表现,病灶无水肿、边缘明显,相位图显示低信号灶中心点状向高信号改变。68例患者检出微出血灶483个,其中微出血灶于基底节、皮下层、丘脑集中最多,脑干、小脑的微出血灶最少;MRI常规序列病灶检出率为9.7%,DWI为35.4%,SWI为100.0%,SWI比其他两种方法检出率更高。结论脑血管病微出血灶中应用磁共振SWI诊断价值较高。 Objective To investigate the application value of magnetic resonance SWI in the diagnosis of microhemorrhagic foci in cerebrovascular disease.Methods 68 patients with cerebral haemorrhagic disease and micro hemorrhage admitted to the Sixth People's Hospital of Xuzhou from January 2019 to December 2021 were selected,and all of them were scanned with MRI conventional sequences and DWI and SWI sequences.The detection of cerebral microhemorrhagic foci in each sequence,the imaging performance of cerebral microhemorrhagic foci in each sequence,different parts of the brain in each sequence and the number of microhemorrhagic foci detected were compared.Results Among the positive detection rates of cerebral microhemorrhagic foci,the positive detection rate of SWI was 100.0%,and its diagnostic results were higher compared with 80.9%for MRI conventional sequences and 94.1%for DWI sequencesthe,the difference was statistically significant(χ^(2)=4.121,14.374,P=0.042,0.001),and the detection rate of DWI sequences were higher compared with MRI conventional sequences,the difference was statistically significant(P<0.05).The SWI sequence showed a small round or oval shaped low signal loss with a diameter of less than 10 mm,but there was no continuity on multiple layers.The focus had no edema and obvious edges.The phase diagram showed that the center of the low signal focus changed to high signal.483 microbleeds were found in 68 patients,most of which were concentrated in basal ganglia,subcutaneous layer and thalamus,followed by brain stem and cerebellum;the detection rate of lesions in MRI conventional sequence was 9.7%,DWI 35.4%,SWI 100.0%,the detection rate of SWI was higher than the other two methods.Conclusion The application of MRI SWI in microhemorrhagic foci of cerebrovascular disease has higher diagnostic value.
