摘要
目的:探讨结直肠息肉与血液流变之间的相关性。方法:选取2020年1月-2022年3月在青海省中医院肛肠科住院行肠镜检查的200例患者为研究对象,根据患者结肠镜检查结果将诊断为结直肠息肉病变患者纳入观察组(n=135),诊断为慢性非特异性结肠炎或正常的患者纳入对照组(n=65),比较两组血液流变学指标水平;再根据病理检测结果将观察组患者分为观察1组(腺瘤性息肉患者,n=69)和观察2组(非腺瘤性息肉患者,n=66),采用Spearman相关分析结直肠息肉与血液流变之间的相关性。结果:对照组血液流变学正常者检出率46.15%、异常者检出率53.85%,与观察组正常检出率17.78%和异常者检出率82.22%相比,差异有统计学意义(P<0.05)。观察组全血低切黏度、全血高切黏度、血浆黏度和纤维蛋白原水平均高于对照组(P<0.05)。观察1组和观察2组全血低切黏度、全血高切黏度、血浆黏度和纤维蛋白原水平均高于对照组(P<0.05),观察1组全血低切黏度、全血高切黏度、血浆黏度和纤维蛋白原水平均高于观察2组(P<0.05)。相关性分析显示血液流变学指标中全血低切黏度、全血高切黏度、血浆黏度和纤维蛋白原四项指标与结直肠息肉间均呈正相关(P<0.05)。结论:结直肠息肉患者存在明显的血液流变指标异常现象,且血液流变学指标异常升高会导致结直肠息肉患者发病率的升高,两者间呈正相关性。
Objective:To study the correlation between colorectal adenoma polyps and hemorheology.Method:A total of 200 patients who underwent the enteroscopy from January 2020 to March 2022 in Proctology Department,Qinghai Hospital of TCM were selected as research subjects.According to examination results,patients with colorectal adenoma polyps were included in the observation group(n=135),patients with chronic non-specific colonitis or normal conditions were included in the control group(n=65).The hemorheology indicators were compared;according to pathological results,patients in the observation group were further classified as the observation group 1(n=69,adenomatous polyps)and the observation group 2(n=66,non-adenomatous polyps).Spearman correlation coefficient was used to study the relationships between colorectal polyps and hemorheology.Result:There were significant differences in the normal and abnormal detection rates between the control group(46.15%,53.85%)and the observation group(17.78%,82.22%)(P<0.05);the whole blood low-shear viscosity,whole blood high-shear viscosity,plasma viscosity and fibrinogen values in the observation group were higher than those in the control group(P<0.05);the whole blood low-shear viscosity,whole blood high-shear viscosity,plasma viscosity and fibrinogen values in the observation group 1 and the observation group 2 were higher than those in the control group(P<0.05);the whole blood low-shear viscosity,whole blood high-shear viscosity,plasma viscosity and fibrinogen values in the observation group 1were higher than those in the observation group 2(P<0.05);correlation analysis indicated that the whole blood low-shear viscosity,whole blood high-shear viscosity,plasma viscosity and fibrinogen were positively correlated with the occurrences of colorectal adenoma polyps(P<0.05).Conclusion:The abnormal hemorheology indicators are generally occurred in colorectal adenoma polyps patients;the rising hemorheology abnormalities may increase the occurrences of colorectal adenoma polyps,and there is
