摘要
目的本研究旨在探索泛素C末端水解酶-L1(UCH-L1)在非小细胞肺癌(NSCLC)组织样本中的表达情况及其临床意义,揭示其作为NSCLC患者预后分子标志物的价值。方法本研究以2017年2月至2019年10月于该院胸外科接受根治性手术治疗的112例NSCLC患者为研究对象,收集患者手术切除后的肿瘤组织与匹配的癌旁组织标本。通过免疫组织化学染色检测UCH-L1在NSCLC组织与癌旁组织中的表达水平,并分析其表达水平与患者临床病理特征的关系。采用Kaplan-Meier曲线和Cox比例风险模型进行生存分析,评价UCH-L1表达水平在NSCLC患者中的预后意义。结果免疫组化染色结果显示UCH-L1在66.1%(74/112)的NSCLC组织中呈不同程度的阳性表达。分析UCH-L1表达水平与肿瘤临床病理参数的关系,结果发现UCH-L1高表达与肿瘤TNM分期(χ^(2)=7.925,P=0.019)、淋巴结转移(χ^(2)=5.311,P=0.021)及组织学分级(χ^(2)=4.177,P=0.041)等侵袭性特征具有显著相关性。生存曲线表明UCH-L1高表达水平者的3年总体生存(OS)率显著低于UCH-L1低表达水平者,分别为39.1%和78.5%,差异有统计学意义(χ^(2)=9.181,P=0.002)。单、多因素Cox回归分析进一步证实UCH-L1高表达水平(HR:3.024,95%CI:1.311~6.973,P=0.009)是NSCLC患者预后不良的独立预测因素。另外2个有意义的预后变量是淋巴结转移(HR:2.092,95%CI:1.012~4.327,P=0.046)和组织学分级(HR:2.153,95%CI:1.062~4.363,P=0.033)。结论UCH-L1高表达水平与NSCLC侵袭性生物学特征及不良预后相关,或许可作为NSCLC患者一种新的预后分子标志物。
Objective To explore the expression level in tissue samples and clinical significance of ubiquitin C-terminal hydrolase-L1(UCH-L1),and to analyze itts prognostic value as a biomarker for non-small cell lung cancer(NSCLC)patients.Methods A total of 112 NSCLC patients who underwent surgical resection At the Department of Thoracic Surgery in Zhangjiagang Hospital Affiliated to Soochow University from February 2017 to October 2019 were included in this study.The surgically resected NSCLC samples and matched adjacent tissues were collected.The expression of UCH-L1 in these tissue samples were detected using immunohistochemical staining,and the correlations between UCH-L1 expression and clinicopathological characteristics of NSCLC were analyzed.Kaplan-Meier curve and the Cox proportional hazards model were used for survival analysis to evaluate the prognostic significance of UCH-L1 expression for NSCLC patients.Results Immunohistochemical staining showed that the positive expression of UCH-L1 were detected in 66.1%(74/112)of NSCLC patients.By analyzing the correlations between its expression and clinicopathological characteristics,we found that patients with high UCH-L1 expression had more advanced TNM stage(χ^(2)=7.925,P=0.019),frequent lymph node metastasis(χ^(2)=5.311,P=0.021)and higher histological grade(χ^(2)=4.177,P=0.041)than those with low UCH-L1 expression.The survival curve indicated that the 3-year overall survival(OS)rate of patients with high UCH-L1 expression was significantly lower than those with low UCH-L1 expression(39.1%vs.78.5%),with a significantly statistical difference(χ^(2)=9.181,P=0.002).Univariate and multivariate Cox regression analysis further demonstrated that high expression of UCH-L1(HR=3.024,95%CI:1.311-6.973,P=0.009)was an independent predictor of poor OS for NSCLC patients.In addition,lymph node metastasis(HR=2.092,95%CI:1.012-4.327,P=0.046)and histological grade(HR=2.153,95%CI:1.062-4.363,P=0.033)were identified as significant prognostic factors.Conclusion High expression of UCH-L
