摘要
目的:探究超声心动图及超声速度向量成像(VVI)技术评估妊娠期糖尿病(GDM)孕妇心肌功能损伤的临床价值。方法:选择2019年4月-2021年3月于本院收治的72例单胎妊娠GDM孕妇为研究对象,根据其心肌损伤情况分为心肌损伤组(40例)和非心肌损伤组(32例);同期单胎妊娠健康孕妇35例作为对照组。均行常规超声心动图和VVI检查,记录常规超声参数和左心室基底段收缩期峰值应变率(SSR)和舒张早期峰值应变率(DSR):Pearson相关性分析心肌酶指标与超声心动图、VVI技术有差异参数的相关性;受试者工作特征(ROC)分析超声心动图、VVI技术参数诊断心肌功能损伤效能。结果:心肌损伤组CK、CK-MB水平均高于非心肌损伤组及对照组,3组LVEF、E/A比较有差异,左心室基底段SSR、DSR在心肌损伤组(-1.61±0.45/s、1.75±0.45/s)、非心肌损伤组(-1.84±0.51/s、1.98±0.52/s)、对照组(-1.99±0.52/s、2.34±0.57/s)有差异(均P<0.05);Pearson相关性分析显示,CK、CK-MB与E/A、基底段SSR和DSR均呈正相关(P<0.05);E/A、SSR、DSR单独及联合评估GDM患者发生心肌功能损伤的曲线下面积分别为0.764、0.791、0.783、0.958,截断值分别为1.25、-1.76/s,1.80/s,联合诊断效果最高(P<0.05)。结论:超声VVI技术监测心肌功能损伤较超声心动图能更清晰、准确评估GDM患者心肌功能损伤。
Objective:To explore the clinical value of echocardiography and ultrasonic velocity vector imaging(VVI)in evaluating myocardial function damage of pregnant women with gestational diabetes mellitus(GDM).Methods:A total of 72 singleton pregnant women with GDM admitted to the hospital were enrolled as the research subjects between April 2019 and March 2021.According to the myocardial damage situation,these women were divided into group A(40 cases with myocardial damage)and group B(32 cases without myocardial damage).And other 35 healthy singleton pregnant women were enrolled in group C during the same period.The women in the three groups underwent routine echocardiography and VVI examination.The values of the routine ultrasound parameters,and the peak systolic strain rate(SSR)and the early diastolic strain rate(DSR)of left ventricular basal segments of the women in the three groups were recorded.The correlation between the myocardial enzyme indexes of the women and their parameters of echocardiography and VVI was analyzed by Pearson correlation analysis.The diagnostic efficiency of echocardiography and VVI parameters for myocardial function damage was analyzed by receiver operating characteristic(ROC)curve.Results:The levels of CK and CK-MB of the women in group A were significantly higher than those of the women in group B and in group C,and there were significant differences in the values of LVEF and E/A of the women among the three groups.Th values of SSR and DSR of left ventricular basal segments(-1.61±0.45/s and 1.75±0.45/s)of the women in group A had significantly different from that(-1.84±0.51/s and 1.98±0.52/s)of the women in group B,and from that(-1.99±0.52/s and 2.34±0.57/s)of the women in group C(all P<0.05).Pearson correlation analysis showed that the values of CK and CK-MB of the women were positively correlated with their E/A value,and their values of SSR and DSR of left ventricular basal segments(P<0.05).The area under the curve of the value of E/A,SSR,or DSR alone,and the combined of the values
