摘要
缺血性脑血管病是神经内科的常见病,动脉粥样硬化是造成缺血性脑血管病的主要病因。介入治疗是临床上治疗缺血性脑血管病的主要治疗方式。目前国内对于颈内动脉颅内段和椎动脉颅内段介入治疗的操作编码存在争议,因此涉及此类操作的编码成为编码工作的难点。根据国际疾病分类手术与操作分类的编码规则,按照脑血管的解剖部位,将颅内外血管介入治疗术式及操作编码进行了总结,以帮助编码员准确分类。支架置入术:颈动脉颅外段00.63、椎动脉颅外段00.64,颈动脉颅内段、椎动脉颅内段、基底动脉、其他颅内动脉00.65;血管成形术:颅内动脉00.62,颅外动脉00.61;经皮粥样硬化切除术:颅内动脉17.54,颅外动脉17.53。编码人员要在掌握ICD基本的编码规则及基础医学知识的同时,通过加强与临床医师的沟通,不断了解医学相关知识的新进展,持续提高编码质量。
Ischemic cerebrovascular disease is the most common disease in neurology.The main reason for ischemic cerebrovascular disease is atherosclerosis.Interventional therapy is a widely used treatment for this kind of disorder.The coding of intracranial interventional therapy for internal carotid artery and vertebral artery is controversial,which makes it a difficulty for coders.After reviewed literatures and surgical records on interventional procedures of ischemic cerebrovascular diseases,we summarized the codes of common interventional procedures and operations of the internal carotid artery,vertebral-basal artery and its important branches to help coders encode accurately.The codes for stent implantation are 00.63,00.64,for carotid artery extracrania segment,vertebral artery extracranial segment,respectively.The code 00.65 is for carotid artery intracranial segment,vertebral artery intracranial segment,basilar artery,and other intracranial arteries.The codes for angioplasty are 00.62 and 00.61for intracranial artery and extracranial artery,respectively.The codes for transcutaneous atherectomy are 17.54 and 17.53 for intracranial artery and extracranial artery,respectively.Medical coders should be familiar with basic coding rules and medical knowledge of ICD.Through constant communications with clinicians,the coders could have a better understanding of the new improvements of medicine,which would improve the quality of coding.
