摘要
目的:采用网状Meta分析评价11种常见干预措施治疗肠造口周围皮炎的效果。方法:计算机检索PubMed、EMbase、Web of Science、the Cochrane Library、中国知网、维普数据库、万方数据库、中国生物医学文献数据库、CINAHL数据库中有关11种常见干预措施治疗肠造口周围皮炎效果的文献,检索时限均为建库至2022年1月。由2名研究者独立筛选文献、提取资料并进行偏倚风险评价,采用Stata 16.0软件进行网状Meta分析。结果:共纳入27篇文献,涉及1 868例病人。网状Meta分析结果显示:水胶体敷料+复方角菜酸酯乳膏、水胶体+藻酸盐敷料、水胶体敷料、水胶体敷料+造口护理粉+皮肤保护膜、壳聚糖伤口护理膜、复方角菜酸酯乳膏、湿润烧伤膏治疗造口周围皮炎有效率均优于氧化锌软膏(P<0.05);水胶体敷料+复方角菜酸酯乳膏、水胶体+藻酸盐敷料有效率优于造口护理粉+皮肤保护膜(P<0.05);根据排序概率表得知,排名前3位的干预措施为水胶体敷料+复方角菜酸酯乳膏、水胶体+藻酸盐敷料、红外线+造口护理粉+皮肤保护膜。结论:现有证据表明,湿性敷料联合干预(水胶体敷料+复方角菜酸酯乳膏、水胶体辅料+藻酸盐敷料)治疗肠造口皮炎的效果更佳。受纳入研究所限,建议未来开展更多高质量、多中心、大样本的随机对照试验进一步验证。此外,建议未来研究应关注不同类型敷料间效果的直接比较及成本效果评价。
Objective:To evaluate the effect of 11 common interventions on perienterostomy dermatitis by network Meta-analysis.Methods:Studies on the effects of eleven intervention methods in the treatment of perienterostomy dermatitis were searched from PubMed, EMbase, Web of Science, the Cochrane Library, CNKI,VIP,WanFang Database, CBM and CINAHL.The retrieval time was from the establishment of the database to January 2022.Two researchers independently screened the literature, extracted the data and evaluated the risk of bias, and Stata 16.0 software was used to network Meta-analysis.Results:A total of 27 randomized controlled trials were included, involving 1 868 patients.Network Meta-analysis results showed that hydrocolloid dressing+compound keratate cream, hydrocolloid+alginate dressing, hydrocolloid dressing, hydrocolloid dressing+ostomous care powder+skin protective film, chitosan wound care membrane, compound keratate cream, moist burn cream were superior to zinc oxide ointment in treating perienterostomy dermatitis(P<0.05);Hydrocolloid dressing+compound ceratate cream, hydrocolloid+alginate dressing has better efficiency than ostomy care powder+skin protective film(P<0.05);The probability results showed that hydrocolloid dressings+compound ceratate cream, hydrocolloid+alginate dressing, infrared+ostomy care powder+skin protective membrane ranked the third.Conclusions:Current evidence shows that the combination of wet dressing and infrared therapy is better in enterostomy dermatitis.Due to the inclusion of the institute, it is recommended to further verify more high-quality, multi-center RCT with large samples in the future.In addition, it is suggested that future studies should focus on the direct comparison of effects and cost effect evaluation between different types of dressings.
