摘要
目的 观察电针对胰岛素抵抗肥胖大鼠胃肠动力和胰岛素敏感性的影响,探讨电针改善胰岛素抵抗肥胖的作用机制。方法 将60只Wistar雄性大鼠随机挑选13只作为正常组给予普通饲料喂养,其余给予高脂饮食喂养进行造模。8周后将造模成功的26只大鼠随机分为模型组和电针组,每组13只。电针组采用电针中脘、关元、足三里和丰隆治疗。干预期间,每隔2周测量大鼠体质量和24 h进食量;第6周检测腹腔葡萄糖耐量实验(intraperitoneal glucose tolerance test, IPGTT)和腹腔胰岛素耐量实验(intraperitoneal insulin tolerance test, IPITT);干预前后,每组随机选取3只行高胰岛素-正葡萄糖钳夹术测定其葡萄糖输注速率(glucose infusion rate, GIR)。干预结束后,心尖取血用酶联免疫吸附法检测血清中胰岛素(insulin, INS)含量;采用BL-420S生物机能实验系统测量3组大鼠胃与小肠肌生物电慢波的振幅和频率;测定胃排空率和小肠推进率。结果 与正常组比较,模型组第0周、2周、4周、6周、8周体质量和进食量均升高,血清IPGTT和IPITT中血糖、INS含量显著升高,GIR显著下降,胃肌电慢波振幅与频率均显著升高,小肠肌电慢波振幅升高而频率降低,胃排空率明显上升,小肠推进率下降,差异均有统计学意义(P<0.01,P<0.05)。与模型组比较,电针组体质量与24 h进食量显著下降,血清IPGTT与IPIT中血糖、INS含量明显降低,GIR显著升高,胃肌电慢波振幅与频率显著下降,小肠肌电慢波振幅降低而频率升高,胃排空率下降,小肠推进率升高,差异均有统计学意义(P<0.01,P<0.05)。结论 电针可以改善胰岛素抵抗肥胖大鼠胃肠动力紊乱,提高胰岛素敏感性。
Objective To observe the effects of electroacupuncture(EA) on gastrointestinal motility and insulin sensitivity in obese rats with insulin resistance and discuss the mechanism of EA in treating obesity with insulin resistance. Method Sixty male Wistar rats randomly contributed 13 as a normal group to be fed with ordinary forage,and the rest rats were fed with high-fat forage for modeling. Eight weeks later, 26 successful model rats were randomly divided into a model group and an EA group, with 13 rats in each group. The EA group received EA at Zhongwan(CV12), Guanyuan(CV4), Zusanli(ST36), and Fenglong(ST40). During the intervention, rats’ body mass and 24 h food intake were measured every 2 weeks;the intraperitoneal glucose tolerance test(IPGTT) and intraperitoneal insulin tolerance test(IPITT) were conducted at the 6th week;before and after the treatment, 3 rats were randomly selected from each group to measure the glucose infusion rate(GIR) using the hyperinsulinemic-euglycemic clamp technique.After the intervention, the level of serum insulin(INS)was detected using enzyme-linked immunosorbent assay(ELISA)with blood collected from the tip of the heart;the amplitude and frequency of gastric and intestinal muscles’ bioelectric slow wave in the three groups of rats were detected by the BL-420 biological function experimental system;the gastric emptying and intestinal propulsion rates were also measured. Result Compared with the normal group, the body mass and food intake increased at weeks 0, 2, 4, 6, and 8 in the model group, the glucose level in IPGTT and IPITT and INS content increased notably, the GIR dropped significantly, the amplitude and frequency of gastric muscle bioelectric slow wave increased significantly, the small intestine muscle’s bioelectric slow wave showed increased amplitude and decreased frequency, the gastric emptying rate elevated markedly, and the intestinal propulsion rate dropped, all showing statistically significant(P<0.05, P<0.01).
