摘要
目的 系统评价全身振动(WBV)训练对老年人下肢移动及平衡能力的干预效果。方法检索Cochrane Library、PubMed、Web of Science、CBM(sinomed)中国生物医学、中国知网、重庆维普、万方等数据库,收集有关全身振动训练(WBVT)为干预方式对老年人下肢功能性移动、平衡能力的随机对照试验(RCT),检索时间截止为2019年12月,文献采取双人独立提取法,由两名研究者按照纳入标准与排除标准独立筛选研究、提取数据及文献质量评价,采用Stata15.1软件进行Mate分析。结果 纳入11篇RCT,共计439例对象,文献质量评价中有5篇高质量研究和6篇低质量文献。Meta分析结果显示:全身振动训练组在计时完成“起立-行走”(SMD=-0.73,95%CI-0.94~-0.52,P=0.000)、5次坐起实验(SMD=-0.44,95%CI-0.70~0.17,P=0.001)用时均低于对照组,在Bery平衡量表得分(SMD=0.97,95%CI 0.66~1.29,P=0.000)、单腿站立时长(SMD=1.33,95%CI 0.71~1.94,P=0.000)方面高于对照组。结论 全身振动训练有利于增强老年人下肢移动与平衡能力,提高自身运动功能,可有效降低跌倒风险。且发现实验组中相对于“仅WBV训练”,“WBV联合运动训练”在老年人移动能力方面干预效果更好,今后可将WBV作为一种“技能化”的辅助干预手段来增加综合训练计划的训练强度。但受文献数量及质量的限制,还需更多高质量研究支持。
Objective to evaluate the effect of whole-body vibration(WBV) training on functional mobility and balance of lower limbs in the elderly. Method to retrieve the Cochrane Library, PubMed,Web of Science, the CBM(sinomed) Chinese biomedical, CNKI, chongqing weipu, ten thousand such as database, collect the whole body vibration training(WBVT) is a means of intervention for the elderly lower limb functional movement, balance the randomized controlled trial(RCT), retrieval time deadline for December 2019, literature take double independent extraction, by two researchers according to the inclusion criteria and exclusion criteria screening study independently, extract the data and quality evaluation, Mate analysis was performed using Stata 15.1 software. Results a total of 439 subjects were included in 11 RCTS. There were 5 high-quality studies and 6 low-quality literatures in literature quality evaluation.Meta analysis results showed that: The total body vibration training group completed the “standing up and walking”(SMD=-0.73, 95%CI 0.94 ~-0.52, P =0.000), five sitting experiments(SMD=-0.44, 95%CI 0.70 ~ 0.17, P =0.001) all lower than the control group, and obtained scores in Bery balance scale(SMD=0.97, 95%CI 0.66 ~ 1.29, P =0.000),and the duration of standing on one leg(SMD=1.33, 95%CI 0.71 ~ 1.94.P =0.000) was higher than the control group, but there was no statistical significance in 6-minute walking test and Tintti scale scores. Conclusion WBV training is beneficial to enhance the elderly’s mobility and balance ability,improve their own motor function, and effectively reduce the risk of falling. In addition, compared with “WBV training only”, “WBV+ sports training” has a better effect on the mobility of the elderly.In the future, WBV can be considered as a “skill-based” auxiliary intervention means to increase the training intensity of comprehensive training plan. However, limited by the quantity and quality of literature, more high-quality research support is needed.
