摘要
目的 :探讨美皮康联合维生素AD滴剂在预防早产儿医源性皮肤损伤中的效果。方法 :回顾分析2020年1月~2021年12月我院新生儿监护室诊疗的早产儿共计300名作为研究对象,按照皮肤护理方式将予美皮康联合维生素AD滴剂护理皮肤的150例早产儿作为实验组,并按1:1比例选取同期行常规皮肤护理的150例早产儿作为对照组。比较两组早产儿医用粘胶损伤、呼吸机鼻塞相关性皮肤损伤、皮肤摩擦伤及液体外渗皮肤损伤的发生率,并使用新生儿/婴儿Braden-Q量表评估干预前后早产儿皮肤损伤风险,使用早产儿疼痛评估量表(PIPP)评估干预前后相关护理操作后早产儿皮肤疼痛情况。结果 :干预后,实验组早产儿医用粘胶损伤、呼吸机鼻塞相关性皮肤损伤、皮肤摩擦伤及液体外渗局部皮肤损伤的发生率均低于对照组,差异有统计学意义。两组胎龄对比,差异无统计学意义,实验组早产儿移动度、活动能力、感知觉、皮肤潮湿状态、摩擦力及剪切力、营养摄入、组织灌注与氧合评分均高于对照组。实验组早产儿PIPP量表各维度评分及总分均低于对照组,差异有统计学意义。结论 :在早产儿皮肤护理中运用美皮康联合维生素AD滴剂,可降低医源性皮肤损伤的发生率,减轻护理操作对早产儿皮肤产生的疼痛刺激,临床疗效显著。
Objective To explore the effects of Mepilex combined with vitamin AD drops on the prevention of iatrogenic skin injury in premature infants. Methods A total of 300 premature infants treated in neonatal intensive care unit of the hospital from January 2020 to December 2021 were retrospectively analyzed as the research subjects. According to the skin care methods, 150 premature infants who received Mepilex combined with vitamin AD drops for skin care were set as experimental group,and 150 premature infants who underwent routine skin care during the same period were selected as control group at a ratio of 1: 1.The incidence rates of medical adhesive injury, ventilator-related nasal obstruction-related skin injury, skin friction injury and fluid extravasation skin injury were compared between the two groups of premature infants, and the neonatal/infant Braden-Q scale was used to evaluate the risk of skin injury in premature infants before and after intervention. Premature Infant Pain Profile(PIPP) was adopted to assess the skin pain of premature infants after related nursing operations before and after intervention.Results After intervention, the incidence rates of medical adhesive injury, ventilator-related nasal obstruction-related skin injury, skin friction injury and local skin injury of fluid extravasation in experimental group were lower than those in control group.There was no statistical significance in gestational age between the two groups. The scores of mobility, activity ability, sensory perception, skin moist state, friction and shear force, nutrient intake, tissue perfusion and oxygenation of premature infants were higher in experimental group than those in control group. The scores of dimensions and total score of PIPP scale of premature infants in experimental group were lower compared with those in control group.
